I loved this article by Forbes highlighting a growing recognition of the need for a healthcare model that addresses the root causes of #healthdisparities. As healthcare professionals dedicated to #advocacy and community engagement, it's crucial to understand the importance of Social Determinants of Health (#SDOH) and Health-Related Social Needs (#HRSN). One key takeaway is the emphasis on "housing first" interventions that provide supportive housing to individuals with chronic health conditions, including behavioral health issues. This strategy and these efforts aim to foster multi-sector collaborations and create a more equitable and holistic healthcare system. Highly suggest diving into this article... #HealthcareInnovation #HealthEquity #PublicHealth #PatientAdvocacy

📅 It’s #SocialPrescribingDay – read our new article by Paul Wilson. 🩺 Social prescribing is the referral of patients to non-clinical treatments – often for mental health needs and long-term health conditions. 🔗 In 2019, the NHS Long Term Plan embedded the role of link workers to coordinate and refer patients from Primary Care Networks. 🏙 Research has found fewer link workers in areas with the highest need – generally urban and deprived communities of lower socio-economic status. 🔍 Public health policymakers at local, national, and regional levels must ensure link worker recruitment is targeted where it can have the greatest impact. 🤝 There is also a need to comprehensively map, fund, and integrate VCSE organisations into primary care, to ensure the sustainability of social prescribing. 👉 Read more: https://lnkd.in/eXYFEfU2 #socialprescribing #publicpolicy #healthpolicy #primarycare

Agreed! Social Determinants of Health include factors that can be the cause of health problems and they can lead to unnecessary hospitalizations, poorer health outcomes, and worse prognosis. Understanding and targeting SDOH is essential for developing long-term protections for communities disproportionately impacted by the healthcare system. Through a holistic view of the problem, we can find a better solution together. #Healthcare #SDOH #Health

Social determinants of health have a significant influence on health outcomes and health disparities for Medicare members. The ability to track SDOH has improved with the use of medical codes, but programs are still needed to help members overcome these challenges, especially for those experiencing social isolation or loneliness.
