CAC (coronary artery calcium) remains an overutilized assessment tool. It's, in fact, incredibly cheap to get the study down but rarely offers much insight into CVD risk or actual disease.
For the right patient with the right clinician it can be a very important tool especially if other means of risk stratification haven't yielded enough data to make the right next decision.
If you're using a CAC to simply decide whether to start a statin especially given that there is already extensive family history of disease, it will be of little value. But for the patient who has intermediate risk then it can be a great decision tool.
