Rural older adults represent 1 in 5 Medicare beneficiaries. The White House has released the first U.S. Playbook to Address Social Determinants of Health (SDOH). Many studies have shown that members of racial and ethnic minority groups experience poorer economic, physical, and behavioral health outcomes due to SDOH. The Playbook outlines an initial set of structural steps for agencies to take to promote equitable health outcomes across communities while emphasizing the need for well-coordinated health and social care systems. The Playbook was released alongside the U.S. Department of Health and Human Services’ Call to Action and Medicaid and CHIP’s Framework. To encourage system transformation and collaboration, partners from the fields of health care, social services, public and environmental health, government, and health information technology organizations are urged to take both individual and collective action by responding to the Call to Action. Value based care from a consumer perspective... This work provides a good starting point for refocusing on the problems that value based care is intended to solve. As I read the article, something that stood out is that VBC is still very small scale focused. If we are going to move the needle on population health, and high quality, cost effective care, we will need to take programs that have demonstrated effectiveness and scale them with much more commitment than has been demonstrated to date.
