⏰ There really aren’t enough hours in the day for a primary care practice, especially for the preventive care that’s become so central to success in value-based care. The pre 5 p.m. sunsets (officially 4:32 p.m. today here at the Wellvana Hub in Nashville) have us thinking about a study published last year in the Journal of General Internal Medicine. It found a solo PCP would need 26.7 hours a day to care for an average patient panel in a traditional fee for service model. And more than half of that time (14.1 hours) would be spent on preventive care alone. 🪄 We want to meet the physicians who have found a way to bend time and space. When patients and PCPs can focus on prevention, the opportunity to improve overall health and well-being thrives. And yet, the latest data shows only about 1-in-4 Medicare patients complete an Annual Wellness Visit each year. Looking beyond Medicare, less than 1-in-10 patients are getting all the preventive care they’re supposed to. #ThankfulThursday, and today, I’m #grateful for Deming’s Deadly Disease #6: “Excessive medical costs. As reported by Dr. Deming in Out of the Crisis (pages 97-98), executives shared with him that the cost of medical care for their employees was amongst their largest overall expenses, not to mention the cost of medical care embedded in the purchase price of what they purchased from their suppliers.” Out of the Crisis was published in 1982. I was introduced to and read it about 5 years later--likely the same time others did as NBC had aired their whitepaper "If Japan Can, Why Can't We?" which prompted the Big 3 auto manufacturers, and others, to retain Dr. Deming for his expertise. How far have we come in the last 42 years to effectively tackle the cost of medical care for employees? Kaiser Family Foundation, portrays themselves as the “leading health policy organization in the U.S.” with the following self-description: “As a one-of-a-kind information organization, we bring together substantial capabilities in policy research, polling, and journalism in one organization to meet the need for a trusted, independent source of information on national health issues—one with the scope and reach to be a counterweight to health care’s vested interests and a voice for people.” The accompanying chart reflects KFF’s research (arrow added by me to highlight the point in time that Deming wrote Out of the Crisis). We simply cannot place a dollar value on life. We can, however, engage individually and collectively to be wise consumers. Employers I have worked within and in serving as clients are often self-insured—willing to take the risk that the combination of offerings to their respective employee group can be drivers to containing or at least combatting ever-increasing costs of medical care. Of the seven Deadly Diseases Deming identified, this one has been amongst the most challenging for HR professionals to navigate in partnership with their respective broker / advisor and senior management team. With the on-going remarkable advancements in medical care modalities and treatments, it’s clear that the cost of care will continue to escalate regardless of how wisely care is utilized. That doesn’t translate, however, into throwing up of hands and disregarding this Deadly Diseases…IMO, it calls for ever-increasing vigilance in identifying means and mechanisms to minimize the impacts of medical care inflation, including meaningfully measurable individual and organizational wellness endeavors that include insight and education for consumers to make informed decisions.
