It’s incredibly freeing to realize that you don’t have to control the people around you to have control of your own thoughts, feelings and actions. For the longest time, I believed that I needed other people to act a certain way to help control my own feelings and emotions; that other people were responsible for what I thought and how I felt. How crazy does this sound when you really think about it?! Other people are autonomous human beings and can think/feel/do anything they want to. What’s in my control is how I think/feel/react to other people. Of course there are personal boundaries that can be set but at the end of the day, it’s not other people’s responsibility to abide by those (not in context of abuse/trauma). Other people do not follow my “policy” for how to act, and how silly you think that they do! Kara Loewentheil is an extraordinary feminist coach who has taught me SO much the last few years. This article regarding ambulatory specific nursing stensitive indicators explains so much of what I believe is lacking in the ambulatory healthcare platform. Acute care teams have well defined metrics and goals to achieve. “Decrease CLABSI rate by X%” or “Decrease patient falls…”. These are much needed and I am glad those teams have these NSIs to measure their patient outcomes. Ambulatory care teams have NO formal metrics or goals to work toward. Healthcare companies attempt to create and “cascade” meaningful goals but often without any ambulatory executive support. At what cost? Not only nursing but all staff in the ambulatory setting feel unheard, unimportant and unsupported.
