“I’m tired” . . . These were the opening words uttered from a patient suffering from co-morbidities in search of answers. Collaborative care is complex and requires interoperability to build comprehensive plan frameworks. The team was diligent in our pursuit, spending several hours making a total of 12 calls to 4 different providers in advocating for the patient’s needs. We need to be better stewards in delivering expedient, integrated solutions for our patient populations.
