Only 20% of health outcomes are influenced by clinical care; the other 80% are influenced by social care, physical environment and health behaviors. I am so proud of the work that our team at Get Well did to launch this large-scale SDOH screening and navigation program to help a client address their patients’ whole health and drive health equity in their communities. Not only are we screening patients for concerns around food, housing, transportation, personal safety, and mental health, we are also guiding patients to the appropriate support resources.
