📑 ANNUAL REPORT | Last year marked a turning point in our mission to improve the care of older adults. With the help of our grantees and partners, we expanded the number of care settings using the 4Ms Framework to more than 3,800, influenced policymakers to introduce the GUIDE Model for dementia care, and supported the dissemination of tools to discuss palliative care. We're paving the way for inclusive, evidence-based care. Let's keep pushing boundaries for the well-being of older adults everywhere. Person-centred care isn’t just an option, it’s an essential requirement when providing care, especially for those living with dementia. Our person-centred Rose Model means that we always see, value and consider the whole person - not just their care needs. For #DementiaActionWeek we have broken down why care should always be person-centred in our latest blog, which you can read here: | https://lnkd.in/eys-ZWsF #DementiaActionWeek #AlzheimersSociety #PersonCentredCare

Did you know that 2 in 3 people living with dementia live at home in the community rather than residential aged care? This means there is a growing need to ensure our first responders have a greater understanding of dementia and its management. The next edition of the Australian Journal of Dementia Care examines this important issue. Subscribe now ➡ journalofdementiacare.com #AJDC #FirstResponders #DementiaCare

Fulltime family care partner. Dementia and Care Partner Advocate. Teepa Snow Certified Independent Consulant and Trainer. Founder of UniquelyMeInitiatives.
I participated in the #CarePartnerSupportSeries (CPSS) in 2021. It was a game-changer for me: 1. The fact that, as a full-time caregiver, I'm at a heightened risk for developing #dementia was a wake-up call, motivating me to prioritize taking care of myself and #riskReduction strategies. 2. One poignant moment in my group was when a fellow care partner shared, "I felt that the group gave me permission to do yoga as a form of #selfCare." That statement stood out for me as it underscored the transformative impact of being in a group of peers on a similar path, offering mutual #understanding and #validation. 3. The Series uses interactive learning cycles starting with an experience, sharing insights, processing them, and making connections. It then invites the care partners to identify personalised solutions that fit their unique circumstances. This method recognises and validates care partners as knowledgeable and resourceful, and it resonated with me a lot. So I became certified in Teepa Snow's Positive Approach to Dementia Care to support others in the same way. I am a big fan of CPSS and I encourage care partners to join the Series! You will be in awe of some of the creative and innovative solutions and strategies that fellow care partners have identified for themselves and the person with dementia they are partnering with. #dementia #Care #carePartner #selfCare #support #PAC #PACMalaysia #personsLivingWithDementia
