But the carceral state also shapes our mental health standards and practices related to physical coercion and containment, diagnostic imposition and indictment, punishment and surveilance, and involuntary interventions--all in the name of "care" that is not so caring. My psychiatric bill of rights ensures that the people I work with never have diagnoses, medications, or interventions forced upon them. Rather, you are the expert of your own life, I strive to be your fiercest advocate, and we avoid further traumatizing you by forcing interventions upon you at all costs. On this Juneteenth, the need to develop a safer, non-carceral response to mental health emergencies, other than calling 911, rings loud and clear. As the recent deaths of Wiz Rosario and Ryan Gainer demonstrate, calling 911 during a mental health crisis can cause irreversible harm. Therefore, the standard recommendation of calling 911 during a mental health emergency is not the safest option. See Wiz Rosario and Ryan Gainer's stories here: https://lnkd.in/gVPMicaC https://lnkd.in/gTTKw_Vb While some guidelines for mental health crisis management at least acknowledge how police involvement carries risk, they do not go into the role anti-Black (and other forms of ) racism plays, nor the risk of police shooting. Furthermore, there is little recognition of how carceral practices persist in emergency room and inpatient settings. What would it look like to set a new standard of care whereby we account for this risk, instead of ignoring it altogether? This effort is already underway through the Antiracism in Mental Health Fellowship, where healthcare providers worldwide are coming together to bridge the gap between clinical care and sociopolitical reality. As healthcare providers, we can no longer stand by with our hands in the air while another death moves in and out of the news cycle. We owe the people more than this. As psychiatric nurse practitioners, we're not only responsible for prescribing. We also support our patients by creating an open dialogue about psychiatric medications. When working with children and their families, psychoeducation is an essential part of your first session -- especially if you're considering adding an antidepressant to their treatment plan. But what makes this decision so hard for many families? Stigma, fear of side effects, the list goes on. All of these concerns are valid, so take the time to talk to your patients about their questions or concerns and educate them about the medication that may benefit their child. The more informed they are, the more active they can be in their child's care and the more collaborative the treatment plan can be. PLEASE READ these extracts from the report entitled Torture in the UK: Following the publication of the Francis Inquiry report, the UK Government, NHS and the CQC in England have taken steps to improve healthcare safeguards and oversight mechanisms. In 2014, a statutory duty of candour was adopted, placing a legal requirement on health and adult social care providers in England to be open and transparent with patients, families and carers when a patient has suffered harm from their care or treatment. A CQC report found, however, that the duty of candour is not working as intended in many acute community and mental health services. NHS trusts in England are now required to appoint a Freedom to Speak Up Guardian to support staff who report concerns about patient care and safety. Although it is too early to assess the effectiveness of these guardians, nearly 4,000 members of staff spoke to their Freedom to Speak Up Guardian up to June 2017, raising more than a thousand patient safety incidents.
