**This has been hard to write, but bear with me...** I have sat with this statement 👇 for a few days, and it continues reverberating with me. How many patient event reports stemming from fear, criticism, bullying, or intimidation must be submitted before someone will listen AND act? 13%-15% of maternal deaths occur on the day of delivery. **Of those, 80% stem from challenges in communication or lack of communication, which are completely preventable.** Psychological safety MUST take a front seat in obstetric care. ✔ Teams MUST be able to communicate and SAFELY have discussions that can result in crucial conversations. ✔ Use of chain of command to support patient safety. ✔ Create environments that support team safety and accountability. 👉 And this is role agnostic...there can be just as many issues with nurses, physicians, providers, and others that become a roadblock to patient safety. If inpatient obstetric team members (including ALL team members) report challenges with communication, reports without actions ➡ organizational control without resolution, and subsequent harm to patients, which is 100% preventable. I will also say that recurrent patient safety issues without resolution can be as traumatic to patient care teams as to the patients themselves.
