🌟 This week's #MeetTheTeam shines the spotlight on Sharon Jackson, one of our global professional instructors. We posed the question: How often should person-centered care plans be updated? Here's her expert insight: Person-Centered Care Plans are just that – person-centered, therefore, how often they should be updated is based on the individual’s needs. Any time important personhood information is discovered, or changes occur, we should adjust their care plans accordingly. Personhood information that is more likely to change in persons with dementia over time include: preferred routines and activities, cognition/dementia stage and the related remaining abilities, and overall health status. In theory, this process sounds simple, but in care settings, it is not as simple as it may sound. That is why we love to partner with organizations who are invested in delivering true person-centered care, as we have training and tools to make this much easier for their teams to deliver. As part of our Dementia Capable Care System and Implementation Solutions, we assist teams to set up systems and tools which make it easy for the direct care partners to document and communicate important personhood information and other changes to the leaders responsible for updating care plans. We train and mentor leaders to establish a schedule to review each of their clients on a regular basis. The goal is to ensure changes are observed, documented in care plans, and shared with the team. Ultimately the goal is to create a Dementia Capable Workforce of trained and skilled team members working together to ensure the individual’s wants, needs and personhood is always at the center of all care planning.
