#ESMO2023- #bladdercancer #urothelialcarcinoma highlights: EV302, CHECKMATE901, THOR, DAD- #DrBradleyMcGregor & I discuss practical implications of the data in advanced disease GU Oncology Now Bladder Cancer Advocacy Network -1) EV+pembro has a practice-changing impact in both cisplatin-eligible and ineligible patients as first-line therapy (median OS 31.5 mo, CR rate 29.1%), 2) Gemcitabine-Cisplatin-Nivolumab should be considered for cisplatin-eligible patients as first-line therapy (CR 22%, Median duration of CR 37 months, finite duration of chemo confers quality of life and cost benefits), 3) Erdafitinib superior to salvage chemo (relevant in clinic) but not superior to salvage pembrolizumab despite numerically better PFS and ORR (less relevant in clinic since PD1/L1 inhibitors have moved to 1st-line space), 4) combination of 2 ADCs, Enfortumab Vedotin and Sacituzumab govitecan promising for further development (non-overlapping targets, payloads and toxicities), 5) Impact of peri-op PD1 inhibition of first-line PD1 inhibitor based combinations and avelumab maintenance needs to be studied
