Rheology is so important. Dermal fillers are not one size fits all. I view most of my fillers as either skin, fat, or bone in a syringe. It helps guide me based on plane of placement and outcome desired. A 34 year old man who had history of sudden loss of vision on evaluation was found to have retinal detachment with giant retinal tear. ‘Dolphin’ in the fundus! An 18-year-old male presented with an oval translucent cystic mass in the inferior nasal peripheral in his left eye as the color fundus (fig. A) shown. High-resolution swept-source OCTA revealed a 10.5 ×5mm "dolphin-shaped" epiretinal cystic hyperreflectivity on cross section image (fig. C) with a little blood flow signals detected in the "cystic wall" (fig. D). A diagnosis of congenital cyst of the vitreous was considered. A 400kHz speed full range swept source OCTA (#BMizar, TowardPi Medical) was used in this case. The excellent optical design of 120-degree ultra-wide field makes it easy and fast to capture peripheral OCTA image. This case was reported in <JFO Open Ophthalmology>, authored by Dr. Xiaohu Liu, team of Prof. Guoping Wang, from Ineye Hospital of Chengdu University of Traditional Chinese Medicine and Prof. Dian Zhao, from Eye School of Chengdu University of Traditional Chinese Medicine. 📃Scientific paper: The emperor has no clothes: OAB can be cured surgically Abstract: The paper by Karjalainen et al., who reviewed 2,933 pelvic organ prolapse surgeries, showed 75% cure for “bothersome” urge urinary incontinence (UUI), is more than an “Aha” moment; it is an “Emperor has no clothes” moment. Since 1976, a convention of “no surgery” for women with UUI (now overactive bladder, OAB) has become almost an article of faith. Yet, surgical cure of OAB has been known since 1997, when this journal published the first urodynamically controlled study with 20-month data: 86% cure for UUI, 85% for frequency, 80% for nocturia following pubourethral ligament (PUL) and uterosacral ligament (USL) sling repair in 85 women. This study was followed by many other publications over the years recording OAB cure. It is not that even a small fraction of the 600 million women on the planet will now undergo surgery, or that damaged ligaments are the only cause of OAB. However, knowing OAB can be cured opens the door for young creative minds to bring hope and relief to these women non-surgically, as well as surgically. Elschnig (Anton Elschnig : Austrian ophthalmologist, 1863-1939) ➡️ Medical Eponyms: ▪️Elschnig pearls: ➖Accumulation of clusters of proliferated epithelial cells particularly seen in post operative cataract patients.  ➖They have the appearance of bunch of grapes or soap bubbles.  ➖May migrate to posterior capsule, continue to accumulate and cause significantly decreased vision. ▪️Elschnig's spots: ➖Elschnig spots are yellow demarcated lesions in the perimacular region that leak fluorescein after occlusion of the choriocapillaris.  ➖When the Elschnig spot heals, a pigment spot is left surrounded by a depigmented pale halo. They are signs of hypertensive choroidopathy. ▪️Elschnig’s scleral ring: ➖is a normal anatomic variation whereby a thin white ring, also known as the scleral lip, is seen immediately adjacent to the optic disc margin.  ➖It is caused by an anterior extension of the sclera between the choroid and optic nerve. ➖The RPE and choroid tissue terminates just short of the optic disc, thus allowing visualization of scleral tissue through the transparent nerve fiber bundles. ▪️Border tissue of Elschnig: ➖The border tissue of Elschnig refers to densely compacted connective tissue that rises up from the sclera to join the Bruch’s membrane and thereby enclose the choroid.
