We all know that an ounce of prevention is worth a pound of cure, but how often do we see this in action in the medical field? This is why I’m really interested in how GLP-1 agonists (obesity and type 2 diabetes drugs from Novo Nordisk and Eli Lilly and Company) will shape our healthcare system. This is a very special class of drugs which not only provide a treatment, but also significant prevention. Obesity is linked to 40%+ of cancers, and heightens risk for many more diseases our there. With GLP-1s treating obesity, I’m confident that their impact will be massive. This new development reminds me of when statins first hit the market. Statist have had a huge impact on our healthcare system and patients, with approximately 92 million people having used them in 2018-2019 [1]. Here’s the kicker though - unlike statins, which help prevent heart disease and stroke, GLP-1s promise to reduce nearly every leading cause of death on the US. Here's the lowdown: #Mounjaro: This diabetes drug, sometimes used off-label for weight loss, belongs to the glucagon-like peptide-1 (GLP-1) receptor agonist family. GLP-1 is a natural hormone that helps regulate glucose levels, slows digestion, and reduces appetite, leading to weight loss. The weight-loss drug semaglutide - commonly sold under the brand names Ozempic and Wegovy - reduced cardiovascular events such as stroke and heart attack in adults with preexisting disease independent of weight loss. It also helped people keep their weight down for at least four years.🌟 Semaglutide is the main active ingredient in the popular weight-loss jabs Ozempic and Wegovy. It mimics the GLP-1 hormone which can signal that your body is full.💪🏻 The analyses looked at the SELECT trial, which included more than 17,600 adults from 41 countries between October 2018 and March 2021 with pre-existing cardiovascular disease and who were overweight or obese. Patients from «a geographically and racially diverse population» took semaglutide or a placebo once weekly for a mean of 3.3 years during the trial. A large-scale retrospective real-world evidence analysis of GLP1 side- -effects in non-diabetic patients shows a 9x increase in pancreatitis and a 3-4x increase in gastroparesis and bowel obstruction. We need a #precisionmedicine approach to weight loss beyond fervent off-label and on-label use of these medications. It is time to curb our enthusiasm. At the Holistic Heart Centers, we launched a metabolic flexibility program to help patients avoid or reduce the need for medications while achieving sustainable weight loss by reversing inflammation and insulin resistance. As a direct follow from my previous post, a real world retrospective study found much higher rates of #pancreatitis, #bowelobstuction, and #gastroparesis in those taking a #glp1 medication for obesity than in those taking #contrave. I don't believe GLP-1 agonists are going anywhere, and I think they will help many people with #obesity over time. At the same time, these risks need to be evaluated further, and we need to try to figure out how to calculate the risk/benefit ratio for individual patients. These medicines will not be for everyone. My prediction is we will continue to find additional benefits and drawbacks over time as we gain more experience with these medications. https://lnkd.in/ek5A3rvN

Could the GLP-1 class of molecules that has been so effective for weight loss have potential to treat patients with Parkinson’s disease? New data published in @NEJM suggest that an older GLP-1 medication called lixisentaide may be effective for Parkinson’s. Patients with Parkinson’s disease who were treated with lixisentaide for 12 months had no progression of motor symptoms while those treated with placebo showed progression. Current Parkinson’s treatments only help with the management of symptoms, but this study suggests that lixisentaide, and perhaps the newer GLP-1 targeting drugs as well, may have potential to slow the progression of this debilitating degenerative disease.
