Congratulations to our customer Porter County Government Health Department for launching "Porter County Find Help" (powered by Findhelp) 👏 Sam Burgett, MSW, LSW, Social Worker with the Porter County Sheriff's Department, founded a Community Resource Network that for the past two years has coordinated with local organizations to list and claim their programs on the site. 🎥 See how they're engaging with local CBOs ➡️ https://hubs.li/Q02Ffwv70 Burgett emphasizes the importance of a customized Findhelp platform: "By having a Porter County-specific page, we can include programs that may not be free or low-cost, providing a comprehensive guide to the community's offerings. Our goal is to prevent residents from reaching the point of crisis before finding the services they need." 📖 Read more about Porter County's work ➡️ https://hubs.li/Q02FfV_P0 #Findhelp #PorterCounty #CommunityEngagement #LocalGovernment

Senior care isn't just about meeting the healthcare needs of our elders; it's about fostering a sense of community and engaging families to ensure the well-being of our senior members. I'm a big believer that creating a supportive community is essential for the well-being of seniors. Many seniors experience loneliness and isolation, which can have a negative impact on mental health and cognitive function. When senior care is a community effort, it can keep seniors engaged with others and give them a sense of joy and purpose. Caring for seniors can also take a toll on the family—it can be a big commitment. When the community steps in to help care for others, it can lift a little of that heavy weight off individual families. I've seen first-hand how communities can improve the lives of seniors and those caring for them. Encouraging neighbors to check in on elderly residents, organizing community events, and establishing senior-focused programs are just a few ways to get the community involved in caring for one another. #SeniorCare #CommunityCare #SeniorLiving

As the authors in this Health Affairs article say, “Considering the inextricable links among #homelessness, poor health outcomes, and increased health care use, it’s time to empower health care leaders to embrace cross-sector collaboration, data sharing for care coordination, and collective action toward ending homelessness in every community.” To support this empowerment, the authors share a number of helpful resources along with lessons learned from the Healthcare and Homelessness Pilot project launched by Community Solutions, Inc. and IHI. #SDOH #HealthEquity #HousingisHealthcare
"Health systems have the power to drive systematic and sustained improvements in the care for people experiencing #homelessness. This system-level work is imperative for healthier and more equitable communities." #SDOH #healthequity #housingishealthcare Health Care's Role In Ending Homelessness | Health Affairs https://lnkd.in/gbnqiijH by Vanessa Davis, Beth Sandor, Hannah Chimowitz, and Lauran Hardin from Kaiser Permanente, Community Solutions, Inc., National Employment Law Project, and HC2 Strategies

Couldn’t agree more with Neil here…if we don’t crack this then we truly will be stuck in a cycle where lack of preventive care and support for people drives them to acute level services earlier than needed, and quality of life is diminished. Our social care services commissioned by the public sector have become all about meeting higher or short term needs associated with much later life stage and higher acuity levels. Our earlier preventive services no longer commissioned despite great outcomes. We are increaingly taking on responsibilities that would have been in the ‘health care’ domain previously, and yet we also see many people in hospitals due to a lack of broader social care needs being met. Preventive care is a serious issue, as much for wider society and economic productivity (impact we are seeing on carer responsibility, their own health and increasing numbers giving up paid work to fulfil caring responsibilities) as for the quality of life, independence and control we would all wish for ourselves for as long as possible. It needs to be seen as such, both politically and financially. We can - and often do - work better together within communities to address some of this but ultimately funding and political will is needed to take this forward on a national level.
The 'pressures' in our hospitals each winter are the symptoms not simply of a lack of social care capacity, but of a systemic absence of focus on what ultimately brought people there and what if left unaddressed, will bring many more us to that point in future. The sad truth is that far too many of us spend longer years alive, but feeling we have little reason to be alive. A sustainable approach to prevention will start there. Part-time activist and freelance writer. Also a Social Worker working in mental health. I also chair BASW England.
I am launching BASW UK (England)'s #HomesNotHospitals today and I feel revolted. The government’s latest commitment was to reduce the number of people with a learning disability and autistic people in inpatient units by 50% by March 2024. They are going again to miss this - and, at this rate, the government won’t meet their own 50% target until at least 2030 at the earliest. 4.9 years is the average length of stay for current inpatients. And let’s be frank about it. Many people are locked up because both local and central government have not invested enough to create the right community support, with suitable housing and skilled social workers - not because of a mental health problem. This is a human rights scandal. Do join us - and imbox me - for some joint working. We must do more and push both local and central government to doing better - and addressing this scandal - quicker and sooner. #BASW

Public healthcare delivery system transformation and behavioral health integration leader. Private practice therapist
This is such a great example of community-based solutions that break through the walls and associated “make them come to us for help” philosophy that underpins our current healthcare delivery model. #communitypsychology #communitymedicine #healthcareinnovation #behavioralhealthintegration
This is what investing in our communities looks like! The growing homelessness and addiction crises requires lots more meeting people where they are and connecting them with the care and services that they need. https://lnkd.in/gAYvPenS

Care navigation and access for the unsheltered aren't where they need to be. Those experiencing homelessness face higher rates of chronic illness, mental health challenges, and substance abuse disorders - and navigating the healthcare system as fragmented as ours is downright brutal. Community Information Exchange Apps (CIEs) can help ease the patient's burden, making it easier for them to securely connect with medical and social providers - while also building a web of providers looking out for and addressing their needs. By securely connecting patients, healthcare providers, social workers, housing services, and other partners, these apps facilitate real-time coordination and information sharing that address social determinants of health and ease the overuse of emergency services. https://buff.ly/3SJRneX

My goodness yes. A systemic issue. The determinants of health show that 36% of health outcomes are driven by the individual yet only 5% of spend goes on interventions at the individual level. Investing in early intervention, education and reducing social isolation will not only improve quality of life but will also reduce crisis in the long term and therefore nhs costs. #prevention #qualityoflife #education
