The notion to "stick it all in the narrative" is sooo NEMSIS V2. Dispatch details, medications, procedures, and more -- all of those should be captured discretely now, the better to: ___________ (a) Reduce charting time dramatically, by as much as 66% -- this is especially true in Texas via Pulsara -to- ePCR integration! (Go on, ask me how I know....) (b) Improve revenue capture -- because every year, agencies do it this way and leave HUNDREDS OF MILLIONS (if not billions) of dollars on the table [maybe we should stop doing it that way, then....]; and (c) The availability of discrete data allows for the construction of CP/MIH and other data-centric programs that require evidence of efficacy over time. Thanks for sharing this accessible read, Wyatt. Having access to the ADT feed in care transitions is pivotal. A lot has changed in healthcare over the last 3 decades: ~Primary Care typically no longer rounds on its hospitalized patients. ~Health information in both the primary care and acute care settings is typically stored in a digital format. ~The specialized roles of inpatient care transitions and primary care case management nursing have evolved and increased in value. ~There is financial benefit for both acute and primary care in preventing hospital admissions and readmissions. Current areas for leadership investment in order to be successful: ~Strong, viable relationships between acute and primary care focusing on aligned, mutual goals. ~Efforts to keep the continuum of care and associated handoffs in the picture. ~Holding teams and partners accountable to data integrity (e.g. clean and current reconciled medication lists) and effective transmission of patient-related information. Requirements: ~Establishment of ideal workflows and handoffs between acute and primary care. ~Communication of ideal workflows embedded in a culture of continual improvement. ~Accessible electronic systems for handoffs. ~Key stakeholder commitment for using data to guide future enhancements. ~Bringing along frontline staff for input and training. It is doable but does require strong leadership, aligned incentives, and a commitment a longer-sighted vision.
