Both platforms telehealth as well as community pharmacies are desperately needed given the overwhelming demand and what's at stake with MOUD. As time ticks by, more lives are lost needlessly. Community pharmacies who'd like a partner to develop said services should be eventually open to consultant telehealth pharmacists leading a helping hand but often lack of IT capabilites and data transparency, EHR etc. to do so successfully. Plenty of opportunity and work to be done in this regard. There are psychiatric board certified pharmacists in every state as well as almost every territory of the US. Think of the potential for collaboration! Yet CSPEN and Scripted platform opportunities currently only remain open to pharmacies and not independent contractor pharmacists. Still love you both though and happy to collaborate if/when you are. Everyone should take pause and think about how continued segmentation of practice and billing (especially restrictive incident to medicare billing and lack of private parity) continue to be a massive barrier to getting pharmacists to every patient that needs one. Pharmacist access is/should be a basic standard of care, not a privilege ❤️ Namaste.
Big news! SAMHSA just made historic changes to opioid treatment programs (OTPs), bringing in much-needed flexibility that was adopted during the COVID crisis. This major shift, the first in over two decades, focuses on removing the (no pun intended) painful barriers to access for patients - largely due to over restrictive regulations. A few key takeaways: - Removes the "one-year" history of OUD requirement - Enhances support for mobile treatment units - Increases flexibility in take-home medication - Establishes permanent telehealth induction of bupernorphine without in-person exams - Broadens the definition of practitioners to include those approved to dispense and prescribe specific medications Interestingly, comments received were around whether community pharmacists could be defined as providers, particularly in areas of physician shortage. The response referenced the elimination of the "x-waiver". So this seems to indicate that the decision will defer to individual states on how they want to consider pharmacists in any future models. I would be curious to hear thoughts from others on this?

Many health systems use “contracts” with patients as part of opioid prescribing for pain management, presenting the risks & asking patients to agree to things like urine drug screening & showing pill usage. However, growing literature points toward the effectiveness of shared decision-making conversations over contracts.
