In the latest edition of TechNation, ECRI shared insights on “Incident Management and Investigation: Mastering the Process.” The cover story features ECRI’s 7-point approach to manage and investigate healthcare incidents from beginning to end. The IMI Plan includes (1) awareness, (2) response, (3) fact gathering, (4) analysis, (5) conclusions (6) recommendations, and (7) legal considerations. “Hospital errors are the third-leading cause of death in the United States. After an incident occurs, it becomes imperative that a comprehensive investigation is completed to prevent recurrence and limit risk to the organization. With time frames in terms of patient contact re the #MHRAsafetynotice still scheduled for next month, research shows that the majority of ASC teams will miss the deadline due to the sheer volume of people to contact, data hygiene challenges and the resources required. We recently hosted a workshop on how automated telephony can help to contact the people affected en masse, identifying those who may be at most risk and prioritising a follow up risk assessment - as well as where data inaccuracies exist. Since the workshop, we have drawn up a business case and designed a process flow diagram, which starts to outline how automated telephony can intelligently address the actions required in the notice (both of which are available on request). Automated telephony has proven success across a range of ASC services from #Reablement to driving the return of #CommunityLoanEquipment, supporting the incoming #CQC inspections and proportionate reviews. We are progressing conversations with a number of councils around the MHRA safety notice challenge - many of whom confirm that the cost of such an approach is far less than the human resources currently being considered or deployed. While FMEA is a valuable tool for identifying and mitigating potential risks, its effectiveness can indeed be compromised by staffing challenges. When healthcare professionals are burdened with heavy workloads and extensive responsibilities, the capacity to rigorously apply the FMEA process may be limited. Organizations must invest in FMEA training and also in addressing staffing challenges. By finding ways to relieve the burden on individuals, such as through workflow optimizations, FMEA can regain its potential as a robust safeguard for patient safety. The synergy between a well-supported staff and an effective FMEA process is the key to a safer healthcare environment.

For this tip of the week, I'd like to talk about hazards. So your use -related risk analysis needs to identify hazards and the clinical harms associated with them, and an important distinction to make for your URRA versus your Risk Management File is that in the URRA, the hazards need to be tied to use of the device.They are Use-Related Hazards, not just hazards themselves. Now, if we pop over to our Use-Related Risk Analysis Work Instruction, I have that same list of hazards expanded just a little bit, because we need to tie these to use of the device somehow. Because, just because the hazard exists doesn't mean it's a Use-Related Hazard.
