AN UNUSUAL CAUSE OF PERIPHERAL LUNG NODULE - NODULAR PULMONARY AMYLOIDOSIS Cone Beam CT guided Biopsy 70-year-old female patient, known case of hypertension and well controlled type 2 diabetes mellitus presented with complaints of cough since 2 months. She had unintentional weight loss of 4kgs over the last year. Her symptoms did not improve with symptomatic treatment and inhalers. HRCT chest performed for further evaluation revealed a 2.1 x 2 cm nodule in right upper lobe, and smaller nodules in the right lower lobe. Under general anesthesia and lung navigation ventilation protocol ‘Cone Beam Computed Tomography’ was performed. The lesion was identified in the right upper lobe posterior segment and marked. Radial EBUS was performed, but no lesion was identified. Using 3 dimensional augmented fluoroscopy a 1.1mm mini cryoprobe was placed abutting the lesion. A repeat CBCT spin showed the cryo probe to be in contact with the lesion. Multiple Cryobiopsies were then performed under augmented fluoroscopy and sent for analysis. Histopathological examination revealed pale, hyalinized, eosinophilic material surrounded by multinucleated giant cells and lymphoplasmacytic cells. This material was found to be congo red positive with apple green birefringence on polarizer. Final diagnosis: ‘Pulmonary Nodular Amyloidosis’ TAKE HOME MESSAGES 1. CBCT can help confirming tool adjacent to lesion even in instances where radial EBUS doesn't show an image (index case) 2. Since the emergence of hydrocephalus shunt therapy in the 1950s, cerebrospinal fluid diversion from the cerebral ventricles to the abdominal cavity (VP shunt/VPS) has dominated with an application frequency of over 70%. In contrast, the alternative drainage from the lumbar region (LP shunt/LPS) was suspected to be more prone to complications. However, in the last two decades, the use of LP shunting has increased significantly. This article shows the current state of knowledge about advantages and disadvantages of the two types of shunting and compares them with each other. Lumboperitoneal shunts offer an alternative in the treatment of communicating hydrocephalus and related diseases - more variety that allows the treating physicians to address their patients' circumstances and concerns.

An association with #congenital_heart_disease and #asplenia or #polysplenia syndromes have been reported. It is important to recognize the enlarged azygos vein at the confluence with the superior vena cava and in the retrocrural space to avoid misdiagnosis as a right-sided paratracheal mass or retrocrural adenopathy. Preoperative knowledge of anatomy may be important in planning cardiopulmonary bypass and to avoid difficulties in catheterizing the heart. PROCARE HOSPITAL

Another Interesting case presented by Dr. Paola García, Cardiovascular Center, CEDIMAT, Dominican Republic: "A 72-year-old female goes to the cardiology clinic due to chest pain and dyspnea. Paraclinical studies are performed that show data of ischemic heart disease, so she is taken to coronary angiography on an outpatient basis, revealing a severe lesion, with moderate calcification, long from the union of the proximal segment with the middle that involves the exit of the 1st diagonal which is a small caliber vessel but supplies an important territory with severe proximal and distal injury; being a Medina classification 1,1,1. We proceed to perform provisional stenting, we treat the 1st diagonal with medicated balloons and after predilatation with conventional balloons the anterior Descending lesion we proceed to place a first EluNIR-PERL stent followed by a second EluNIR-PERL stent overlapping the anterior one without any difficulty and less contrast due to the stent markers, even ending with post-dilatation of the stent guided by markers". #Innovation #EluNIRPERL #PatientOutcomes #HealthTech #cardiovascular #coronary #Medinol #MedicalDevices #Cardiology #Innovation #CardiovascularHealth #DrugElutingStent #StentInnovation #Medinol #HealthcareInnovation #PatientOutcomes #HealthTech #cardiovascular Smeling Baez
