We are thrilled to announce the publication of our latest research in BMJ Open Quality, titled "Reducing Hospital Length of Stay through Geographic Cohorting and Multidisciplinary Rounds: A Retrospective Study at Duke University Hospital." In the United States, hospital length of stay (LOS) has been increasing since the onset of the COVID-19 pandemic, leading to numerous negative outcomes such as decreased quality of care, worsened patient satisfaction, and adverse financial impacts on hospitals. While many factors contributing to prolonged LOS are difficult to modify, poor coordination of care and communication among clinical teams are areas where improvement is possible. Our study proposes geographic cohorting of provider teams, patients, and other clinical staff as a solution to prolonged LOS and readmissions. Despite mixed results from previous studies on geographic cohorting alone, our comprehensive approach integrates additional interventions to enhance its effectiveness. We conducted a retrospective study on the General Medicine service at Duke University Hospital, implementing a bundled intervention that included: • Geographic cohorting of patients and their care teams • Twice daily multidisciplinary rounds • Incremental case management support Our findings were significant. Patients in the intervention arm experienced 16-17% shorter LOS compared to those in the control arms. Additionally, there was a noticeable reduction in 30-day hospital readmissions compared to the concurrent control arm. We also observed some improved accuracy in estimated discharge dates in the intervention arm. Based on these encouraging results, Duke University Hospital has recognized the value of geographic cohorting and has implemented additional geographically-based medicine units with multidisciplinary rounds. These changes aim to improve hospital throughput and patient outcomes, reducing LOS while enhancing the quality of care provided. We believe this study provides valuable insights and a potential pathway for other institutions looking to address similar challenges. Future research will focus on confirming the sustained impact of these care transformations. Saving nearly a day is incredible. Are you looking to expand this into the resident physician population, and if so, how do you think it will reflect and the overall like the state numbers? My feeling is that residents may be able to incorporate this system much easier.

It's not because of covid 19, it's due to insurance companies denying of prior authorization and hospital leadership not knowing how to conduct prior authorizations in a way that gets approved quickly..
