Island Health Care (IHC), a federally qualified health center on Martha’s Vineyard, addresses healthcare challenges for residents, 8.8% of whom live at or below the poverty line! Despite the recognition of community health workers' importance over the years they often lack digital tools & are often disconnected from traditional healthcare technology systems.
