Restrictive Visitation Policies and Related Post-Traumatic Stress Among Families of Critically Ill Patients With COVID-19. Although most participants remembered ways that restrictive visitation policies made their ICU experience challenging, the perceived impact of these policies and whether these memories were a source of traumatic stress differed for families of deceased vs surviving patients. Most participants who had experienced symptoms of PTSD had lost a loved one in the ICU and believed that visitor restrictions contributed to the patient’s death. Among family members of surviving patients who experienced symptoms of PTSD, their symptoms were not clearly related to memories of restrictive visitation policies. The belief that the patient’s death could hace befen avoided had there been family presence in the ICU was common among family members who experienced symptoms of PTSD. After the ICU stay, some families may need support to identify and reprocess unhelpful behaviors such as self-blame and perceived helplessness. In the future, we should consider whether the perceived societal benefits of restricting visitation during a pandemic outweigh the risks of emotional distress to families, particularly when the disease carries a high risk of death. https://lnkd.in/eySf9ysR #medicine #criticalcare #icu #publichealth

(aka molly, aka ecstasy) 💊 The drug is seeking FDA approval, paired with psychological intervention, to treat PTSD. 🧠 If approved, it could mark a shift in US drug policy, recognizing MDMA's medical potential and rescheduling it. This is HUGE, and would be one of the few times a Schedule-I substance is moved into a less restricted category! “My psychiatrist says I have dissociative symptoms. Am I losing my mind?” 🗨 #NormativeDissociation, commonly referred to as absorption, is widely distributed in the general population. Whereas #ClinicalDissociation symptoms are less common and more debilitating. So, #dissociation is a wide spectrum.. but it does not mean you are “going crazy”. 🗨Feeling more absentminded than before, having regular episodes of numbness, etc., can be attended to with some strategies but sometimes more is required. With increasing severity of symptoms, more factors need to be formulated and accounted for in treatment planning. But when does dissociation become a clinical condition? 🤔 *** 🔬 What does the latest research say about this? 🔎 A study examining a U.S. sample found that childhood maltreatment strongly predicted severe dissociative symptoms such as amnesia, depersonalization, and identity alteration. These presentations can be treated but by a multi-disciplinary team of professionals well-versed in dissociative trauma work. The study has also highlighted that the condition can turn for worse under untrained care. 👨🔬👩🔬 We recommend those with debilitating symptoms to seek clarity with a psychiatrist and explore medical treatment alongside psychotherapy. https://lnkd.in/gj7FyA_w This past week, Dr. Shereef Elnahal, VA Under Secretary for Health, led a candid discussion on psychedelic assisted therapies for Veterans experiencing a number of mental health conditions. Joining him in this discussion was Jon Lubecky, a Marine and Army Veteran, who participated in a MDMA-AT research trial and has been an advocate for expanding research and access to these novel therapies, Dr. Ilse Wiechers, Deputy Executive Director of VA’s Office of Mental Health and Suicide Prevention, and Dr. Joshua Wooley who is one of VA’s leading researchers on psilocybin-assisted therapies. Some highlights: 🌿Jonathan Lubecky talked about how conventional medications and psychotherapies including Cognitive Behavioral Therapy and Prolonged Exposure did not help to resolve his PTSD. Although he acknowledged that these treatments do help some Veterans, his experience with MDMA-AT was a resounding success and “when it comes to mental health, all options need to be on the table”. 🌿VA Under Secretary for Health Dr. Shereef Elnahal: “If and when this comes available, VA will be able to scale it better than any other healthcare system because of our focus particularly on Veterans.” 🌿Dr. Wiechers & Dr. Wooley both shared the sentiment that there are too few Veterans in published studies, and that there needs to be further research focused on VA patients within the VA system treated by VA providers. Research will provide answers to questions such as are these therapies safe for those with certain comorbid conditions, who it will be effective or ineffective for, and how to best optimize the treatment for Veterans.

Here is our latest medical paper on stellate ganglion blocks (SGB), which has published medical evidence for treating PTSD, anxiety and Traumatic Brain Injury (TBI). In this paper we report on a rare event in two patients experiencing increased anxiety after doing an SGB on the left side, AND HOW we were able to improve their symptoms afterwards. This research is so critical, especially for oncology patients who are at a greater risk of facing early traumatic experiences. It is imperative clinicians learn and understand the impact a cancer diagnosis has on a patient with PTSD, and how it affects a patient's decision making, and care pathway. Implementing a trauma informed approach to care and treatment may improve both the patient experience, and health outcomes.
