Collaborative Safety Senior Director, Kelly Knutson’s MSW, M.Sc. thesis at Lund University explores the application of systemic incident analysis methodologies, specifically the AcciMap approach, to maltreatment investigations in licensed facilities. The study focuses on how this approach can support investigators in understanding complex systemic interactions and identify systemic influences to prevent future incidents. By facilitating AcciMap sessions with licensing investigators, the study demonstrates that this method helps move away from blame, provides a comprehensive view of incidents to enhance prevention efforts, and supports key aspects of investigative work. The findings suggest that systems thinking and subsequent systemic incident analysis methods can offer valuable insights and improvements in handling maltreatment cases, differing from traditional linear investigation models that can limit learning and improvement efforts. Site feasibility questionnaires (FQs) only contain a small percent of information relevant to the Principal Investigator (PI) and the protocol, the rest is capability or performance data from the site. So why aren't we reusing the information we already have?
