We vehemently oppose the proposed $125 million cut to the Health Home program in the SFY 2024-25 New York State proposed Executive Budget. This proposed decimation, shrouded in an alarming lack of specifics, jeopardizes a program critical to the well-being of 180,000 high-risk, high-need adults and children. These individuals, often battling complex health challenges including mental illness, substance abuse, and chronic conditions, rely on Health Homes for coordinated, community-based care that extends far beyond mere medical treatment. Health Homes are not impersonal institutions, but lifelines woven into the very fabric of our communities and health care ecosystem. Dedicated care managers, embedded in neighborhoods they understand, deliver culturally sensitive support that addresses not just medical needs, but also the health-related social care needs that can profoundly impact an individual's well-being. This holistic approach, meeting people where they are most comfortable, is fundamental to the program's success. Governor Hochul's own vision, eloquently articulated in her budget presentation, champions expanding services, improving access to care, and protecting the vulnerable. Her commitment to mental health, opioid crisis support, children’s services, housing, and community-based services directly contradicts this callous cut. On the cusp of major transformation under the newly approved 1115 Waiver Amendment Health Homes will be more important than ever to ensure NYS is successful in meeting the ambitious goals outlined in the waiver and more importantly ensuring the New Yorkers identified receive services.
