“When I think of #wellness in terms of pharmacy, I really think…about what we were saying around [the idea of] when you think of a pharmacist, wellness is typically not the first thing that comes to your mind. You may think white coats and medications and things like that, but when you think of wellness as a part of #pharmacy, I look at it more from a prevention standpoint, as well as synergizing with current patient care. So anything we can do to either prevent further chronic disease states or anything like that and also take on the management side. We say, “Okay. What can we [wellness pharmacists] do in addition to your current medication and your current therapy to really help you [the patient] with your personal #wellbeing?”
It can make a difference in how a patient feels and their overall quality of life. So, Advanced Care Planning is like having a crystal ball for your healthcare future. It's all about having those oh-so-fun discussions about what you want your medical care to look like down the road, especially when you're hitting the golden years or dealing with a chronic condition like CKD.
Now, here's the kicker—many folks wish they had known about this party earlier. You know, like the one you show up to fashionably late and realize everyone's already left. Yeah, that feeling. Turns out, having these chats before the crisis hits can save you a boatload of stress and ensure your wishes are respected.
Picture this: you're laid up in a hospital bed, unable to voice your preferences. Your family's squabbling over what to do next, and you're stuck in the middle like a kid watching their parents fight over the last piece of pie. Not exactly the dream scenario, right?
But wait, there's more! Advanced Care Planning isn't just about you—it's about sparing your loved ones from the agony of making tough decisions on your behalf. Think of it as your final act of kindness, the ultimate mic drop moment.
And hey, let's talk paperwork! No, seriously, it's not as boring as it sounds. Designating a healthcare proxy and filling out those legal forms might not be the highlight of your week, but trust me, it's worth it. You get to choose someone to be your medical spokesperson, your superhero in scrubs, if you will.
Now, let's address the elephant in the room—end-of-life care. Yeah, it's about as cheery as a rainy day at the beach, but discussing your preferences early can lift a weight off your shoulders. It's like ordering your favorite pizza ahead of time—you get exactly what you want when the craving hits.

Dr. James Reason noted, “Rather than being the main instigators of an accident, operators tend to be the inheritors of system defects created by poor design, incorrect installation, faulty maintenance and bad management decisions. Their part is usually that of adding the final garnish to a lethal brew whose ingredients have already been long in the cooking.”
In healthcare, this often means that the caregiver unmasks the latent failure mode hidden in the system that has been "waiting" to breach the last slice of Swiss Cheese resulting in injury to patient, family, and caregiver. Consider care that a caregiver provides and a patient experience that fails to add value/advance the cause of wellness. Further, consider that this care may be "routine/the way we have always done it" and, thus, patients continue to experience this care...until it is challenged as to how it adds value by the team.
However….relying on the bedside caregiver to challenge the care before it reaches the patient requires awareness of this care (voice of patient that may be hidden in the background noise of care) and requires human performance as discussed in: -> Leonard M, Graham S, Bonacum D The human factor: the critical importance of effective teamwork and communication in providing safe care BMJ Quality & Safety 2004;13:i85-i90 -> Noted:
“A large and ever present cultural barrier is the deeply embedded belief that quality of care and error free clinical performance are the result of being well trained and trying hard. In this paradigm, inevitable mistakes are viewed as episodes of personal failure, with the predictable result that these events are minimized and not openly discussed. Human factors science tells us that the inherent limitations of human memory, effects of stress and fatigue, the risks associated with distractions and interruptions, and limited ability to multitask ensure that even skilled, experienced providers will make mistakes.”
Imagine moving away from relying on a “well trained” caregiver “trying hard” to eliminate this non-value added care; Imagine providing the bedside caregiver with systems and tools that make it easy to do the right thing and provides barriers to non-value added care hidden in the “the way we have always done it” from reaching the bedside (patient and caregiver).
