To start with process safety one has to identify the right tools and how to use them! The requirements are very important for the quality of analysis for any process industry. Some methodologies like Preliminary hazard analysis are used to get a broader picture and to identify which hazard needs to be studied and analysed in depth depending upon the severity. As a Safety Professional, everybody comes across Herbert Heinrich Theory (1931) and Frank Bird Theory (1966) i.e. Accident Causation Model. Still in many organizations, these theories are being used and management tries to superimpose the different particulars on the triangles to reach to a certain conclusion or derive a particular assumption. I have few questions (which I can’t ask to Mr. Heinrich and Mr. Bird):- a.      What industries or sectors were being referred or considered for establishing both the theories, considering the gap of 35 years? b.     Are both the triangles still relevant for every type of industry? For Example, A Big Construction site and a Huge Petrochemical complex or Chemical Process Plant. c.      Is investigation of Near-Miss recorded, was considered before the theory was established? Points to Ponder:- a.      Can an Industry or an organization establish their own Safety Triangles (Accident Causation Model) based on the numbers they recorded in the past years, which will enable them to anticipate an undesired event in a specific way to prevent it? b.     Is correlation between the different particulars which were being taken in above two theories still have a same impact or some addition and deletion is required, to make it more reliable?
