Community Supports are essential services for Medi-Cal members, focusing on providing well-rounded care beyond a doctor’s office or hospital. At CenCal Health, we are committed to helping our members with their health-related social needs. These needs are often tied to issues like food insecurity, inadequate housing, and transportation barriers. Through Community Supports, we are working with our partners to positively impact health outcomes and overall wellness for the people we serve in San Luis Obispo and Santa Barbara counties. Starting today, we are expanding our Community Supports with our partners at Access Tlc Caregivers, Oxnard Family Circle ADHC, Independent Living Resource Center (ILRC) and Libertana. Here is what's new: - Asthma Remediation - Community Transition Services/Nursing Facility to a Home - Environmental Accessibility Adaptations (Home Modifications) - Nursing Facility Transition/ Diversion to Assisted Living Facilities (Residential Care Facilities for Elderly and Adult Residential Facilities)

Arizona is home to many retirees, and the number of older adults living here is growing every day. With this population boom comes an increased need for high-quality, affordable care services that meet their individual needs. Smaller residential facilities such as assisted living communities are a great way for seniors to receive personal assistance with activities of daily living (ADLs) in a home-like setting. Assisted living communities provide a safe, secure environment for seniors and peace of mind for families. Residents can maintain a sense of independence while receiving help with tasks such as bathing, dressing, personal hygiene, meal preparation, housekeeping, and transport. I want to be in an ordinary atmosphere because I think that helps me.” John on getting treatment at home with nurse Annie through the Integrated Care for Older People (iCOPE) programme, as we continue to focus on our Healthy Lives Partnership week. iCOPE focuses on proactive care of people aged 65 and over. Livewell Southwest is working with Primary Care Networks and voluntary sector partners to identify frailty and support patients’ wishes for their ongoing care. Each GP practice holds regular meetings where therapists, social workers, GPs and frailty practitioners discuss the needs of patients such as medication reviews, social support networks and advanced care planning. This is helping reduce the amount of time older people are spending in hospital along with improving their quality of life and life expectancy. #HealthyLivesPartnership #IntegratedCare

I was fortunate to facilitate a dialogue with Traditional Health Practitioners, the elderly and persons living with disabilities. Our aim was to understand the personal experiences of these persons in relation to the health, social and economic impacts of sinkholes. Here are some interesting things that were shared: 1. Building medical centres or clinics specifically offering traditional healthcare 2. Having medical aid pay for consultations with traditional health practitioners 3. Displacement of elderly persons has disrupted the family unit. Families cannot ensure the safety of family members 4. Limited access to healthcare as healthcare facilities and roads have falled through the ground 5. The provision of durable medical equipment (such a wheelchairs) as they break easily when used outside. You’d be surprised by the number of people who use wheelchairs as a mode of transportation to travel long distance. 5. No economic activity causing an increased community dependence on government assistance, increased illiteracy, increased burden of disease and an increase in poverty. Even in the early stage discussions of increasing user pay in aged care, the national conversation appears to be already having an impact. Despite not being our target audience at all, Community Home Australia is already receiving enquiries for full fee for service admissions (our first user pay guest will be moving in soon) and we have had 5 new private pay members join our programs in the last 3 days. While the sector is heartily applauding the introduction of more user pay to fill the coffers, are they actually inadvertently starting the great movement away from institutional care? There is not a nursing home in the country that can match us for staffing ratios, educational standards, community connectedness, meaningful engagement or person centred care. Clearly people don't mind paying, but expect something more than mediocrity and institutionalisation.
