HF is the leading cause of hospitalization among older adults and Medicare beneficiaries with HF have the highest readmission rate of any condition. It's estimated that nearly half of these hospitalizations can be avoided by optimizing outpatient therapy. Retrospective research has indicated that patients with Type 2 Diabetes had an adherence rate of 62-64% to their insulin treatment. Another study involving 144 adults revealed that 59% forgot to take their insulin, with 46% reporting non-adherence. Acute care costs and outpatient costs have been found to be significantly lower for adherent patients. PAMAN makes medication adherence easy and simple! https://paman.org.uk/home/ #DiabetesManagement #MedicationAdherence #HealthcareInsights

Hospitalists often encounter patients with opioid use disorder (OUD) but may lack the knowledge to assess patients with suspected OUD, manage withdrawal, initiate treatment in the hospital, or link patients to care in the outpatient setting. Hospitalists and other hospital-based clinicians require the knowledge and strategies to competently treat, fluently facilitate patient-centered conversations about treatment and support care transition plans and safe discharges. In partnership with ORN, the Society of Hospital Medicine developed tools to assist hospitalist-led teams in the implementation of interventions to improve the care of hospitalized patients with OUD with the use of FDA-approved medications. An implementation guide with a step-by-step approach to support the implementation of targeted strategies to improve the care of hospitalized patients with OUD is now available here: https://bit.ly/47hFEsJ #OpioidResponseNetwork #SocietyofHospitalMedicine #OpioidUseDisorder #AddictionTreatment

“Unraveling the contrasting perspectives of healthcare providers and caregivers highlights the intricacies of these conditions [Angelman syndrome & SYNGAP1]. Physicians prioritize clinical terms, while caregivers emphasize life-altering symptoms. This divide underscores the need for comprehensive symptom reporting, which will be a crucial step towards early suspicion and diagnosis in rare conditions." There's a clear divide between physician and caregiver-reported symptom frequencies for those living with #AS & #SYNGAP1. Postural Drainage: an old but still very useful way to mobilize thick, retained secretions. Add Percussion to that and you have a very effective mucus clearance therapy that spreads along professional therapists from Respiratory to Physical therapists. 🤯 Some mind-blowing statistics to consider on hospitalizations and readmissions... 🤯 On average, the cost of a diabetes-related inpatient hospital stay is estimated to be between $9,000 - $12,000. (Source: Healthcare Cost and Utilization Project - HCUP) 🏥 And, the estimated cost of preventable readmissions is approximately $17 billion annually in the United States. 💰 💰 A notable portion of diabetes-related hospitalizations is considered preventable. Studies suggest that up to 79% of hospitalizations among patients with diabetes for uncontrolled diabetes and its complications could potentially be prevented through effective outpatient care, disease management, and patient education. (Source: Agency for Healthcare Research and Quality - AHRQ) 📊 What would you give to reduce hospitalization or readmission rates by 50%? Would it surprise you to know that #Florence has demonstrated reductions as high as 66% in patients with diabetes? 📉 Interested in learning more? I would be, too!
