📃Scientific paper: The accuracy and robustness of plasma biomarker models for amyloid PET positivity Abstract: Background Plasma biomarkers for Alzheimer’s disease (AD) have broad potential as screening tools in primary care and disease-modifying trials. Detecting elevated amyloid-β (Aβ) pathology to support trial recruitment or initiating Aβ-targeting treatments would be of critical value. In this study, we aimed to examine the robustness of plasma biomarkers to detect elevated Aβ pathology at different stages of the AD continuum. Beyond determining the best biomarker—or biomarker combination—for detecting this outcome, we also simulated increases in inter-assay coefficient of variability (CV) to account for external factors not considered by intra-assay variability. With this, we aimed to determine whether plasma biomarkers would maintain their accuracy if applied in a setting which anticipates higher variability (i.e., clinical routine). Methods We included 118 participants (cognitively unimpaired [CU, n  = 50], cognitively impaired [CI, n  = 68]) from the ADNI study with a full plasma biomarker profile (Aβ42/40, GFAP, p-tau181, NfL) and matched amyloid imaging. Initially, we investigated how simulated CV variations impacted single-biomarker discriminative performance of amyloid status. Then, we evaluated the predictive performance of models containing different biomarker combinations, based both on original and simulated measurements. Here, we aim to synthesize the experiences of clinicians leading preclinical AD biomarker disclosure.

#Artificial urinary biomarker probes for diagnosis:- •Urinalysis is a widely used medical test for healthcare monitoring and disease diagnosis. However, traditional urinalysis relies on endogenous biomarkers that have limited diagnostic sensitivity and specificity. •To address these issues, molecular optical probes have been engineered to interact with disease biomarkers in vivo and produce artificial urinary biomarkers (#AUBs). •AUBs are then excreted into urine for the remote detection of diseases through urinalysis. In this Review, we first introduce AUB probes and highlight the benefits of AUBs over endogenous urinary biomarkers. •We then discuss the design principles of two categories of these probes, namely, intrinsic AUB probes and AUB-secreting nanoprobes, with their corresponding detection modalities in urine test. •Finally, we summarize the applications of AUB probes in disease diagnostics and discuss the current challenges and strategies to advance their clinical translation. #Keypoints:- •Current urinalysis tests using endogenous biomarkers suffer from low biomarker concentration and poor specificity. •AUBPs are a new class of molecular imaging probes designed to interact with disease-associated biomarkers, which release AUBs that are remotely and readily detectable in urine. •AUBPs consist of intrinsic probes and AUB-secreting nanoprobes tailored to different disease biomarkers, administration routes and in vivo pharmacokinetics. •AUBs possess unique optical, mass, biological and catalytic properties, which render AUBs distinguishable from endogenous substances in urine.

Biomarkers that will allow an accurate diagnosis need to be established, and studies searching for and evaluating biomarkers are underway worldwide.4, 5 The Biomarkers Definitions Working Group defines biomarkers as properties that are objectively measured and evaluated as indicators of normal biological processes, pathological processes, or pharmacological responses to therapeutic interventions. In general, these include vital signs used in daily practice, as well as various laboratory and imaging data, such as those from biochemical tests, blood tests, and tumor markers. The expression of genes (DNA and RNA), proteins and peptides, or imaging modalities (such as CT, PET and MRI) are used to measure these biomarkers. For example, to evaluate useful diagnostic biomarkers, the ADNI (Alzheimer’s Disease Neuroimaging Initiative) study examined the most predictive biomarkers of mild AD by performing a variety of psychological, imaging, and cerebrospinal fluid tests. “How can science and medical innovation support cancer patients?” This is the question that several experts answered at the THRPTX, the Therapeutics Innovation Summit organized by Servier on June 18, 2024. Collective intelligence The participants in this first roundtable highlighted the importance of adopting a holistic and multidisciplinary approach to meet the major challenges with oncology, including a personalized approach to medicine. The development of targeted therapies, the drive to tackle mechanisms of resistance to treatments and the optimum use of breakthrough technological innovations such as AI are paving the way for more personalized medicine. Pooling resources and expertise across all the stakeholders will make it possible to significantly improve the results of treatments, accelerate their development and increase the accessibility of health care over time. Working closely with patients Each patient is unique, and this obvious statement highlights some of the major challenges with research. During these exchanges, the experts called for patients and patient associations to be involved in the early stages of innovation, from research to therapeutic protocols. Not only is their knowledge of the disease valuable for researchers, but also these “co-creation” dynamics will position them to be actively engaged and help drive research. Precision medicine, which involves tailoring medical treatments to the characteristics of each patient group, is one factor for ensuring a concrete response for patients. By improving the way differences between individuals are taken into consideration, particularly for clinical trials where certain population categories are still underrepresented, we will be able to develop solutions that are increasingly adapted to individual molecular characteristics. Lastly, the discussions highlighted the importance of making changes to the criteria for assessing treatments, by taking into account improvements in patients’ living conditions during their treatment for instance.
