I’ve been a “knucklehead.”
❤️🩹Thank you, Timothy Gilligan, for this critically needed piece. To all in my network, this is a must-read.
Patients each have a story; autonomy and decision-making cannot be judged without context. We ought to consider, with structured intent, concordance, and caregiver competency to hear that story.
⁉️ Why is it that upon logging into the EHR, there is no “card” (a digital post-it?) right at the top of the chart that allows for patients to tell us who they are and what we should know when caring for them. As a peds nurse I came to call this (on my shift report sheet) the “Is Bobby afraid of the dark?” section. There are things we should know when we are caring for others beyond what pushed them into the setting.
🥜 During a hospitalization last year, someone wrote & stated I had delusions of being in a Ph.D. program and that I had invented the screen savers on the inpatient desktops. (Women in STEM with disabilities, can you believe we exist? 🦄) The Vital Scout early warning system was my Master’s work with William H. Morris, MD, MBA, leading the former Clinical Solutions Center.

The resident didn’t listen when my family implored “Google… please.” When you are labeled a “knucklehead,” no one will hear what you have to say - even when your story is on the hospital’s own website. (https://lnkd.in/dUzFWrqb)
We cause harm we do not measure. I started to believe the hospital team over my own family, staring at my own hard-fought academic work in the hospital room, questioning my very reality. This sort of thing occurs every day - especially to marginalized patient groups.
We CAN change it.

Those “knuckleheads” just don’t always do what we want them to do…”what surprised me about these two patients is that no one seems to have heard their stories during the initial encounters with the healthcare system. The only story that was heard was the story of the tumor.”
“This is what I do know: I am not entitled to my patients trust. It is something I have to earn each time I walk into the room.”
Work hard to stop labeling. Let's ask better questions. Period.
