Hello, friends! As vice-president of data science and advanced analytics at Unity Health Toronto, Dr. Muhammad Mamdani has led the implementation of over 50 AI-powered solutions into clinical practice. These solutions range from an early warning system using electronic medical records to predict a patient's risk of death or requiring intensive care to a brain-bleed detection tool that fast-tracks access to critical treatment. And according to Mamdani, there's more to come in 2024. "I hope to see more AI being used for clinical decision making," says Mamdani. As a professor in the department of medicine at the University of Toronto's Temerty Faculty of Medicine and director of the Temerty Centre for Artificial Intelligence Research and Education in Medicine (T-CAIREM), he is well-positioned to know. However, Mamdani cautions that AI is not a panacea for the underlying problems in Canada's health system. Despite its potential to transform patient care, AI must be used judiciously and with care. Mamdani holds cross-appointments in the Leslie Dan Faculty of Pharmacy and the Institute of Health Policy, Management, and Evaluation at the Dalla Lana School of Public Health. Next week, I will head to Salt Lake City, Utah, to attend the 22nd International Conference on Artificial Intelligence in Medicine (https://lnkd.in/gpJpuhRN). I will present two oral presentations based on the outcomes of my recently concluded PhD thesis: The first presentation, titled "Checklist to Support the Development and Implementation of AI in Clinical Settings," will be part of the International Workshop: AI Applications in Telemedicine and Digital Health (TeleHealth-AI). The second presentation, titled "Framework to Support the Design and Development of AI Systems for Clinical Settings," will be presented during the workshop on Implementing AI in Healthcare: Bridging the Gap Between Technical, Cognitive, and Sociotechnical Perspectives. Today in Medical Economics, Dr. Michael D Abramoff and I write about the need for a new science to address the growing need to scale medical AI. We discuss our first-of-its-kind randomized controlled trial with Orbis International, which shows a 40% increase in clinical productivity using autonomous AI for diabetic eye exams, underscoring AI's potential to transform resource-constrained healthcare. Thank you Yuri Quintana, PhD, FACMI, Steve Labkoff, MD, AMIA (American Medical Informatics Association), the DCI Network, and all the organizers for inviting me to participate in the "Blueprints for Trust: Best Practices and Regulatory Pathways for Ethical AI in Healthcare" conference and workshop at Harvard Medical School. It was an honor and pleasure to be part of such a select group of healthcare professionals, researchers, industry leaders, and policymakers as we devised new models for the trusted use of #AIinHealthcare for the #FutureOfMedicine. Our work has only begun in creating a usable framework to inform health policy on the safe and equitable use of AI within healthcare, but several important points stood out: • There are human consequences of bias in AI, and the right type of dataset is fundamental to ensuring the best quality of care possible for every unique patient. • Real-world evidence is at the very least multimodal, unstructured, and nuanced, and all factors must be considered and accurately captured. • AI-enabled clinical decision support systems demand trust from its end users, and trust is gained through thorough system understanding and competence. • Patients have the right to know how AI-enabled systems are being used throughout the delivery of their healthcare, and educating patients in a user friendly way on AI system use is indispensable. • Healthcare is not a single process nor a one size fits all journey, context matters and AI-enabled systems need to be appropriately tailored and not simply scaled up. • Humans are, and must always remain, the key decision-makers of all aspects of patient care.
