Some of you may know that I spent the majority of my career working with value-based care programs.  Since making the switch to life sciences, I’ve realized that the final WHY of new guidance doesn’t always make it back to the folks working in early stages of clinical trials. I hope this article helps close the loop for many on why PROs are increasingly important and scrutinized in clinical trials. As my colleague John Fryer points out, “It’s not about carving out populations, it’s about delivering these healthcare models in their totality across all populations that we collectively serve." Clinicians have mixed panels of patients, and they cannot be expected to think differently about certain sets of people in a value-based context and not others. The partnership between Lumeris and NS-EEH represents a meaningful step forward in the quest to move to a more value-oriented model of care. Germany, Switzerland, Netherlands and Jaoan and other Democratic capitalistic nations ensure access for all their citizens to quality affordable private insured and private fee for service manufactured medical care at a fraction of the cost of America’s insurance centric healthcare mess with better outcomes and costs than in America. I have had several conversations this week and it has gotten me thinking about how part of the work we do in the name of value-based healthcare needs to be more about creating connective tissue for patients and families to remain the center of their care and decision making at every point in their health or illness journey. Without connective tissue care is isolated and reverts to individual professional opinion or succumbs to the mercy of constraints within the circumstance. This does not feel compassionate, empathetic, patient-centered, or value-based. Instead, it feels isolated, abandoned, with a sudden loss of autonomy and support when those 2 aspects of agency are most needed. In essence we (healthcare) drops the ball when we do not think of care delivery as an ecosystem of care, but instead as a service contained to a certain space. This scenario can happen at any point of care, but frequently occurs with transitions of care and end of life care, when patients and families have the least amount of information, resource awareness, or sense of community support, and also have the greatest amount of disconnect with lower health literacy about landscape and options within these care segments. Building infrastructure within our systems of health based on the premise of patient-centered care and having ongoing "runway" conversations with patients and families going through significant illness about personal values and priorities of importance gives lead time to process information and prepare creating the necessary care links, allowing patients to remain in control of their most precious right, the right to remain in control of their health with autonomy,  dignity, partnership, and connection. Both in and out of the walls of healthcare environments. This is how value-add care looks and feels to a patient and family ... compassionate, accessible, connected, and personal. Regardless of the terminology used, ultimately each us desires care for ourselves and loved ones that confirms our needs are being met. To get to this level of care we need to have conversations that lead to care that is thoughtfully designed and masterfully delivered. Creates connection between patient - provider-community - and payers. This kind of care always costs less and yields immeasurably more. "Great fragments don't necessarily add up to a great system.
