Developing healthcare access paradigms that give advocates in rural and other underserved communities simple, cost effective strategies that help to overcome social, cultural, geographic and institutional barriers to care is what HeartSense is all about. We must not ignore the inextricable link between public health and long term treatment of CV disease, which begins with early diagnosis and navigation. First and foremost, we must be PRESENT in the community and empower existing community health infrastructure to touch their constituents. It’s not just a month that is needed for this but March is Endometriosis Awarness Month. Symptoms of endometriosis include pain in your lower tummy or back (pelvic pain), severe period pain and pain during or after sex. It's not clear what causes endometriosis. It may be linked to things like your genes or a problem with your immune system. One in 10 women of reproductive age in the UK suffer from endometriosis. 176 million worldwide. The prevalence of endometriosis in women with infertility is as high as 50 per cent. #iamthatone #endometriosis #endometriosisawareness

Join us as we initiate crucial discussions surrounding endometriosis during this month. We aim to shed light on the challenges faced by individuals living with endometriosis, increase understanding among the public and healthcare professionals, and advocate for better support and resources. Some live key takeaways from our webinar with Caitlin Synovec from National Health Care for the Homeless Council: ✔ Hospitals can provide consistent, predictable funding for programs through contracts and rates. ✔ Even in states where other funding mechanisms exist or are in development, hospital funding supports operational costs and non-billable services. ✔ Medical respite can contribute to hospital goals such as decreased length of stay and reduce readmission rates.

A large majority of maternal death rates are preventable. Access to information supporting healthy births and mapped digital care journeys linking in-person visits to digital experiences should be the norm for expecting mothers from pre-pregnancy through postpartum. It's time we build a more consumer-centric health system that actually makes people better. What a great partnership to tackle a huge issue.
📢 Announcing a new, impactful collaboration: Pager and League have teamed up to tackle the maternal health crisis. It's no secret that the current healthcare system is not meeting the needs of expectant mothers. Together with League, we are stepping up to offer convenient, trusted clinical care, safe transportation, personalized educational resources, and a strong network of support. Equitable access to care is possible. Learn more about the power of this collaboration and a new standard for maternal healthcare: https://lnkd.in/eMMHdpta

Delighted to share this incredible story on the importance of the Patient Activation Measure (PAM) in healthcare! PAM serves as a robust tool for evaluating patients' readiness and ability to manage their health effectively. By assessing factors like knowledge, skills, and confidence, PAM enables healthcare providers to tailor interventions and support to meet individual patient needs. This personalized approach not only fosters greater patient engagement but also leads to improved health outcomes and overall satisfaction. Let's continue collaborating and leveraging innovative solutions like PAM to enhance patient-centered care and drive positive change in healthcare delivery. #PatientActivation #HealthcareInnovation
HealthLinc serves more than 40,000 patients annually in northern Indiana. Like many FQHCs, HealthLinc has limited resources—and their community health workers (CHWs) needed a better way to support patients with social needs. So, in partnership with Phreesia, HealthLinc implemented the Patient Activation Measure® (PAM®) to help better understand the kind of support its patients need. Now that PAM is integrated into its workflows, HealthLinc’s CHWs have been able to connect 43% more patients with relevant SDOH resources. Watch this video to hear the HealthLinc team discuss how a partnership with Phreesia has made a difference for the organization and its patients.
