Yesterday, I spent nine hours in A&E with my mother (she’s fine!). Aside from the sobering reminder of the NHS’s current struggles, I was fascinated by the role patients were playing in the provision of healthcare. Whenever a doctor or nurse called for a person who did not appear, it was the patients in the waiting room who informed them that that person has gone to x-ray, that person had been called for bloods… There was a healthcare gap, and with nothing better to do than sit and wait, the patients seeking healthcare filled it. I’m not saying the waiting room patients were wrong - and I was definitely involved! But the situation did make me reflect on how we relate to the systems around us. It is so easy to become caught up, and part of, existing systems – no matter how dysfunctional they are. We are hardwired to find clear solutions, to try to make things work, and if the quick fix means confidentiality is abandoned and doctors start relying on patients to keep track of other patients, then that’s what we’ll quickly adjust to. We want to keep going from where we are, rather than stopping and reflecting on how we are. At Making Every Adult Matter (MEAM), we know the importance of remaining curious about the systems we encounter. We know that rushing in and creating simple ‘add-ons’ to existing systems doesn’t work, and can cause bigger problems in the long-run (what about when lack of confidentiality causes a serious issue? When your patients in the waiting room aren’t well enough to keep track of other people?). Patience and perspective are some of the greatest tools we can use when dealing with systems, to help us get to the root of the problem. Rather than rushing in, and suggesting plugs for those obvious gaps, we’ll question what those obvious gaps reveal about the system itself, considering all the components involved and the connections between them (doctors, nurses, admin staff, patients, patients’ carers, etc…, as well as processes and structures). This approach is essential when working with multiple disadvantage, where all too often the “quick fixes” result in longterm harm to an individual’s life, through missing an essential opportunity for care to eroding their faith in services. Taking a whole systems approach is what enables us to make meaningful and sustainable change. Thanks to the attention of the NHS staff, I was glad to get my mum home safely (and to a proper cup of tea!) but couldn’t help reflecting on how once you’ve bought into systems thinking you start to see everything differently - even in the most unexpected situations.
