Wraparound Care Coordinator - Youth & Family Department

Columbia Community Mental Health is a nonprofit agency and Certified Community Behavioral Health Clinic that provides services to individuals with mental, addiction and developmental disabilities.

The Youth and Family Department is looking for someone with a growth mindset who is driven, curious, and passionate about serving youth and families in their community to be our next Wraparound Care Coordinator. This individual will be providing services in home, community, healthcare, and school-based settings for youth and families who are struggling to have intensive mental and behavioral health needs met by their current service providers and natural supports.

Position: Wraparound Care Coordinator

Department: Youth and Family

Shift: Mon-Fri 8:00 am to 5:00 pm

Salary Range: $27.31/hour - $35.31/hour. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

Position Requirements: Requires a QMHA: a bachelor's degree in a behavioral science field from an accredited college or university or three years of full-time professional experience working in the mental health related field. Note: All QMHA positions in the state of Oregon will need to be filled by candidates who are officially QMHA certified or registered with MHACBO. Summary

The Wraparound Care Coordinator is responsible for coordinating/supporting behavioral health care within CCMH and the referrals to clinically indicated services and supports outside of the agency. Provides direct client services.

Working conditions: This position works in an office environment as well as in the community, with about 70-75% of the work done in the community. This position requires driving to different locations within Columbia County and some occasional driving to locations elsewhere in the Portland Metro. The Care Coordinator will also be available to provide services in the client's residence as needed.
