The Registered Nurse position assists with day-to-day nursing activities. The RN works collaboratively as an active member of the interprofessional team. Systematically and continuously collects and assess data related to patient health status to develop, execute and evaluate the plan of care. Work independently within their scope of practice to achieve clinical outcomes. This position requires accuracy, attention to detail, and a passion for providing exceptional healthcare.
Utilize data to continuously meet care coordination needs across settings in conjunction with the patient, family, and/or other care providers.
Assume responsibility and accountability for effectively managing nursing care of individual patients.
Document nursing assessments and interventions to reflect comprehensive and integrated approach to patient care.
Assist or perform procedures (e.g., ECGs, catheter changes, injections, etc.)
Perform telephone, My chart and in-person triage for multispecialty ambulatory clinic.
Complete chart prep and paperwork, including FMLA, CMN forms, etc.
Assist with medication management (e.g., medication renewals)
Attend daily huddles, monthly PCS meetings, monthly clinic meetings, CCM and RWHC meetings, etc.
Serve as a mentor and resource to others.
Perform other duties as requested.
EPIC/Healthlink Experience Preferred.
Position may require week of orientation to be performed in Madison, WI.
