We are seeking a highly skilled and experienced Vice President of Population Health to join our team. As the Vice President, you will play a crucial role in leading and managing our organization. You will be responsible for ensuring that patients receive the right visit at the right time by the right provider, that staff are productive, that appropriate tools are used to promote proactive healthcare, and that care teams are appropriately staffed. This includes monitoring and promoting the appropriate use of tools such as chronic care management, transitional care management, advanced care planning, screening for cognitive behavior, labs, x-rays, and other screening tools. This is an excellent opportunity for a dynamic leader with a strong background in data analytics, expertise in value-based care and population health management.

- Provide strategic guidance and leadership to all departments within the organization
- Develop and implement effective business strategies to drive growth and achieve organizational objectives
- Conduct research and data analysis to identify trends, opportunities, and challenges in the industry
- Collaborate with cross-functional teams to develop innovative solutions and drive continuous improvement
- Foster a culture of collaboration, teamwork, and excellence throughout the organization
- Managing cost containment across the organization to successfully achieve the goals of being attributed to an Accountable Care Organization.
- Stay updated on industry trends, regulations, and best practices to ensure compliance and drive innovation
- Bachelor's degree in healthcare administration, social work, nursing, or related field; Master's degree preferred
- Minimum of 7-10 years of experience in case management or related field, with at least 3 years of leadership experience.
- Proven experience in a leadership role, preferably as a Vice President or similar position
- Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization
- Strong problem-solving abilities with a strategic mindset
- Ability to work effectively under pressure and meet tight deadlines
- Demonstrated track record of driving results and achieving targets
