We’re based out of Des Moines, Iowa, but we’ve got teammates spread across the US in many departments. Day to day, we collaborate most frequently using Slack, Notion, Zoom, and Figma.


About the Position

The CMA will support patients in achieving their health and wellness goals, with an emphasis on lifestyle modifications, exercise routines, dietary changes, and health-related reminders. This role offers an exciting opportunity to make a meaningful impact on patients' lives while contributing to the success of our innovative care delivery model.


Responsibilities

  • Patient Engagement and Support:
  • Collaborate with CMAs to develop and implement personalized care plans for each patient, tailored to their individual needs and preferences.
  • Conduct live video consults with patients on a monthly basis, providing empathetic and supportive guidance to help them achieve their health goals.
  • Assist patients in setting achievable lifestyle and exercise goals, monitoring progress, and adjusting plans as necessary to promote success.
  • Offer encouragement, motivation, and emotional support to patients throughout their wellness journey, ensuring they feel valued and empowered.
  • Communication and Coordination:
  • Collaborate with the CMA and No Patient Left Behind (NPLB) teams to ensure seamless coordination of care and timely follow-up on patient needs or interventions.
  • Care Plan Development and Management:
  • Assist in the creation and refinement of personalized care plans for patients, incorporating feedback from clinical leaders and leveraging health coaching principles.
  • Utilize software tools and AI-driven solutions to streamline care plan development and enhance communication between care team members.
  • Work closely with patients to review and iterate on care plans over time, adjusting goals and strategies based on their progress and evolving health needs.
  • Continuous Learning and Improvement:
  • Participate in ongoing training and professional development activities to enhance knowledge and skills related to care coaching and patient engagement.
  • Contribute to the continuous improvement of our care delivery model by providing feedback, insights, and suggestions for optimizing the patient experience


Requirements

  • Current CMA certification
  • Minimum of 2 years of experience in healthcare, with a focus on patient engagement, health coaching, or chronic care management.
  • Strong interpersonal and communication skills, with the ability to build rapport and trust with patients from diverse backgrounds.
  • Empathetic and compassionate demeanor, with a genuine desire to help others achieve their health and wellness goals.
  • Familiarity with health coaching principles, behavior change techniques, and motivational interviewing strategies.
  • Comfortable using technology and software tools to facilitate virtual consultations, communication, and care plan management.
  • Passion for promoting healthy lifestyle behaviors and improving patient outcomes.
