About the Role: HopeLink Behavioral Health is seeking a compassionate and motivated Supportive Housing Specialist to join our team. In this role, you will play a critical role in assisting individuals with serious mental illness find and maintain permanent housing, utilizing Housing First principles. Working under the supervision of the Supportive Housing Supervisor, you will develop individualized recovery plans, provide supportive services, and collaborate with a team of professionals to ensure successful outcomes for our clients. Hybrid, Supportive and collaborative work environment. Monday -Friday 9am -5pm Responsibilities: Conduct assessments to determine clients' needs, strengths, and stage of recovery. Engage clients in skills-building activities focused on recovery, community integration, and wellness. Assist clients in securing rental assistance, finding housing options, and adhering to lease agreements. Develop and implement person-centered treatment plans and document services provided. Collaborate with clients to make informed decisions about their mental health recovery. Liaison with other care providers and treatment team members. Maintain electronic health records in accordance with CARF, licensure, and Medicaid regulations. Demonstrate flexibility and willingness to perform additional duties as assigned. Represent staff in agency and community workgroups focused on mental health support services. Qualifications: Bachelor's degree in Social Work, Psychology, or a related field (preferred) Qualified Mental Health Professional (QMHP) certification (preferred but not required) Minimum of 2 years of experience working with individuals with serious mental illness and housing issues Excellent written, oral, and interpersonal communication skills Strong computer skills and proficiency in relevant software programs High degree of empathy and a commitment to serving individuals with mental illness Ability to work independently, manage a caseload, and meet deadlines Ready to Make a Difference?
