Description

The Support Care Manager is part of a multi- disciplinary team that assists our clients to achieve their recovery and parenting goals while engaged in the Pregnant and Parenting residential program at Wellmore Behavioral Health. The Support Care Manager is responsible for identifying areas of need and connecting individuals with services in the community to address these needs. Responsible for scheduling and coordinating appointments for women engaged in the program and for their children. The Support Care Manager will act as an advocate for clients and assist individuals develop life skills to support success upon discharge.

Examples Of Responsibilities

  • Oversight, planning and coordinating transportation for clients to ensure safety and compliance with medical appointments for themselves and their children, connection to community resources and basic needs. Assisting with and ensuring supervision of clients off site during these trips and appointments as appropriate.
  • Facilitating Psycho-Educational group sessions with clients. Topics to help clients achieve their treatment plan goals including but not limited to; Parenting, household management, budgeting, job readiness and obtaining basic needs.
  • Work with the treatment team to ensure all case management needs identified for client are obtained for the client and her child(ren). Responsible for obtaining resources, connection to other programs and other needs are met.
  • Assist with residential components of the programs. Including but not limited to; supervising clients throughout the shift to ensure client safety. Including client counts, supervised drug screening, supervised smoke breaks and client down time. Support clients in completing their activities of daily living (ADLs) for themselves and their child.
  • Support the Technicians in providing childcare for client’s engaged in the program while they are participating in group or individual services. Assist directly to provide childcare when Technicians are completing other job requirements. Assisting and supporting the child with age-appropriate play and activities.
  • Responsible for submitting and maintaining clinical records in a manner that assures compliance with all agency policies and procedures. Maintain clinical documentation that meets all departmental, divisional, agency, local, state, and federal expectations, and regulations. Complete and maintain documentation within identified agency software.
  • Develop effective networking relationships with outside contractors, referral sources and community agencies. Engage in ongoing marketing of programs and represent the agency at community events and meetings as assigned by Management.
  • Provide coverage as needed for co-workers during vacations, holidays, and/or periods of absence. Helps to ensure that the program is fully covered at all times.
  • Participate in supervision, team meetings, rounds, agency committees and work groups as assigned by management. May be required to participate in trainings (both on site and off site) as determined by the Director of Residential Services and/or Vice President of Adult Services.
  • Complete other duties as assigned.

Qualifications

Associate degree/two years of college in Human Services related field is preferred. Experience as a substitute may be considered. At least six months of experience working with cases similar to those served and working in a Residential setting is required. Training preferred on the following topics: Treatment Criteria for Addictive, Substance-Related, and Co-Occurring Conditions (ASAM), Co-occurring disorders, including prevalence, signs/symptoms, assessment, treatment and the impact of one’s own attitudes/beliefs on the delivery of services and the use of naloxone in response to an opioid overdose.
