POSITION

Sr Clinical Manager

POSITION PURPOSE

  • Maintain nursing protocols, including:
    • Clinical protocols, ensure that they are current and reflect best practices in home health
    • PASCO administrative policies and procedures regarding proper communication, process, paperwork requirements, etc.
    • Individualized protocols for waiver services clients
  • Interact with peers and manager to create, refine and execute on cross-department processes and solve escalated issues
  • When appropriate, support outreach efforts by representing PASCO as a clinical expert, at events as well as pre-intake calls with potential new consumers. This may take place during or after business hours.
  • Perform other duties and responsibilities that are assigned, or required to fulfill listed key duties
Supervises waiver services nurses and serves as lead for other Clinical Managers to ensure that the care delivered is high quality, meets or exceeds regulatory standards, is financially sustainable, and results in satisfied consumers. Manages waiver services nurses’ orientation, training, schedules and performance, and works with peers to identify, create and implement solutions to escalated issues.

Key Duties and Responsibilities

  • Manage waiver services nurses’ overall performance and schedules.
    • Oversee the orientation of new nurses and support nurses to ensure they are trained and progressing appropriately.
    • Assign nurses to clients, distribute case-loads, make changes based on client or nurse needs and preferences or changes in nursing staff, ensure all nursing visits are covered, review accuracy of community care nurse timesheets.
    • Review referrals to determine appropriate nursing coverage.
    • Notify nurses of hospitalizations and ensure incident reporting and follow up is complete.
    • Notify Medical Records and/or designee of upcoming discharges.
    • Manage schedule and performance of on-call nurse program.
    • Receive and prioritize client calls and complaints regarding nurses as well as proactively contacting clients to gather feedback.
    • Provide constructive feedback and coach nurses.
    • Proactively manage and complete waiver services nurse performance evaluations in a timely manner.
    • Communicate information to the waiver services team regarding company updates, leadership initiatives and regulatory information, as well as communicate waiver services information back to the administrative team and provide input to leadership on department needs.
    • Support and motivate waiver services nurses, regularly check in with staff, discuss any issues and provide information, resources, and support to nurses who need it.
  • Ensure delivery of high-quality care.
    • Ensure nurses consistently exhibit appropriate clinical skills and knowledge and oversee regular skills competency testing.
    • Oversee completion of periodic waiver services reports, including but not limited to infections and incidents.
    • Proactively analyze quality of all care and identify areas for improvement or action, with specific focus on those areas identified in the Quality Management Plan.
    • Lead Advocacy and Forward Looking Care initiatives on behalf of waiver services clients.
    • Ensure all orders and plans of care are followed by nurses and staff members, monitor order frequency for nursing visits and ensure appropriateness.
    • Ensure that nurses are analyzing effectiveness and outcomes of care, and initiating necessary changes in consultation with physician.
    • Ensure that nurses are acting as a liaison between PASCO, physicians, inpatient and outpatient facilities, and other community resources.
    • Ensure that nurses are providing effective care coordination across all care providers.
    • Serve as a clinical expert and resource for escalated care delivery questions, provide or closely supervise those services requiring substantial and specialized nursing skills, and proactively stay on top of emerging techniques or practices in nursing and home health.
    • Lead quality improvement initiatives – determine needs, and develop and implement solutions.
  • Ensure optimum delivery of care in accordance to rules and regulations, including (but not limited to) Colorado Department of Public Health Standards, Medicare Conditions of Participation, Colorado Health Care Policy and Financing, the Nurse Practice Act of Colorado, and HIPAA.
    • Hold nurses accountable for timely and accurate paperwork.
    • Develops and implements processes and education for nurse and CNA staff in the field.
    • Ensure that community care nurses understand the applicable rules and regulations, and are performing accordingly; oversee orientation as well as on-going education
    • Proactively work with Quality Assurance to monitor nurse performance and address issues.
    • Investigate incidents and complaints, report according to rules and regulations.
    • Participate in QAPI and QMP Initiatives, initiate quality improvement initiatives, identify trends, execute a plan, review data, improve outcomes.
  • Review all incoming clinical faxes for ROCs/labs/orders/visit notes, distribute to case managing nurses and medical records and follow up on any associated tasks.
  • Follow up with nursing staff to ensure completion of required documentation according to rules, regulations, and agency policies and practices.
  • Participate in challenging conversations with clients and complete follow through for behavior contracts, discharge notices and warning letters.
  • Lead appropriate meetings.

Sr Clinical Manager will adhere to responsibilities as defined in the Conditions of Participation:

  • The Sr Clinical Manager will provide oversight of all client care services, including but not limited to:
    • Making client and personnel assignments.
    • Coordinating client care.
    • Coordinating referrals.
    • Assuring that client needs are continually assessed, and
    • Assuring the development, implementation, and updates of the individualized plan of care.
  • The Sr Clinical Manager will be available during operating hours.
Key Measures of Performance for the Role

  • Governing body survey findings and outcomes.
  • Quality of care metrics (e.g. infection rates, wounds, etc.)
  • Timeliness and accuracy of nursing paperwork
  • Consumer feedback on nursing staff
  • Consumer retention
  • Community care audit results
  • Engagement, morale and retention of employees, especially community care nurses

Requirements

  • Experience:
    • 2+ years of Home Health nursing (or equivalent) required
    • Experience managing nurses within a Home Healthcare company preferred
  • Education and/or Licenses:
    • Current license to practice as a Registered Nurse in the state of Colorado
    • Bachelor’s degree in nursing preferred
    • Master’s degree in related field preferred
  • Knowledge, Skills or Abilities:
    • Thorough to expert clinical and logistical knowledge of home health care service delivery
    • Thorough to expert knowledge of Medicare, State & Licensure Home Care Agency and any other governing body rules and regulations for care delivery and documentation
    • Knowledge of state Medicaid waiver programs
    • Leadership and management skills, e.g. ability to monitor progress, audit performance and provide effective feedback, influence, motivate, manage conflict and change
    • Strong to expert clinical skills, ability to provide or act as a resource in cases of substantial or specialized nursing skills
    • Appreciation of how all areas of the business work together and affect each other; specifically, the short- and long-term ramifications of clinical decisions on PASCO’s operations, finances, advocacy and regulatory relationships
    • Ability to communicate effectively and proactively, ensuring that all members in the organization have accurate information in a timely manner
    • Ability to identify, assess and prioritize legal, financial and clinical risks in their domain, as well as efficiently minimize, monitor and control those risks
    • Strong organization and time management skills, ability to maintain one’s own schedule and meet deadlines
    • Knowledgeable and capable with Microsoft Outlook, Word, Excel, PowerPoint, and experience with Electronic Health Records systems
    • Ability to work with a diverse population
  • Physical or Other Requirements:
    • Medium detailed work:
      • Requires walking or standing a significant amount of the time.
      • Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
      • Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing.
      • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).
    • Access to a reliable transportation, either public transportation or an automobile with a valid Colorado driver’s license and insurance.
    • Ability to work weekends / on-call on a rotating basis.
