Responsibilities

The Social Work Supervisor’s professional responsibility within their scope of practice: abides by and enforces all compliance requirements and policies and performs these responsibilities in an ethical manner consistent with the organization's values; adheres to hospital policies including confidentiality; works the assigned schedule including overtime and call as required; enhances professional growth and development through participation in education programs; Requires minimal supervision to safely perform essential functions; utilizes proper body mechanics when performing essential functions; maintains and operates equipment and reports broken or malfunctioning equipment; supports all personnel as valuable assets to the institution; upervises Social Worker staff in all work-related functions while providing 24/7 support; assists in maintaining the clinical documentation system for the care management department; assesses for best resource allocation with patient management activities for both the acute and post-acute settings; identifies and reports appropriate discharge barriers and avoidable delays to promote a resolution process with the healthcare system; provides assessment for the source and significance of social, psychological, environmental, emotional, and economic factors that may contribute to the health and response to treatment of both inpatients and outpatients of all ages and documents findings in medical record per department policy; prepares, attends, and presents committee meeting materials; actively coordinates patient and family interventions with the healthcare team, patient, and patient support system; develops a plan of intervention using an interdisciplinary approach to establish continuum of care in congruence with ethical and legal considerations; advocates, medicates, and negotiates a cohesive plan for maintaining or improving social supports and patient safety; maintains a positive working relationship with healthcare team and community agencies to better serve the patient population; evaluates the effectiveness of interventions and actions in relation to the intended goals of the discharge plan and revises intervention strategies as needed; identifies barriers in service delivery systems and advocates for changes to improve patient outcomes; assures chart documentation of social worker staff consistently meets department and health system standards for accountability and confidentiality; addresses issues or areas of concern within the scope of social work practice prior to escalating to the Director/Assistant Director; maintains current knowledge for federal and state regulatory guidelines, institutional policies, and payor source policies; attends appropriate clinical and professional organizational workshops, conferences, and meetings in efforts to stay abreast of community resources available to facilitate safe patient transitions of care; remains current on clinical advancements related to primary patient population and proactively seeks understanding of all roles in the department; maintains the schedule for Social Worker staff as needed; performs payroll functions for Social Worker staff as needed; assesses applications, performs interviews, and hires candidates in conjunction with case management leadership; monitors performance metrics including productivity, quality, and key department indicators. Creates action plans to address outliers; performs annual performance evaluations as well as documenting performance and addressing staff who require progressive discipline; knowledge and Application of Trauma Informed Care Principles; knowledge and Application of techniques for trauma related issues such as grief, loss, adjustment issues and crisis counseling; ensuring effective utilization of services within the department including transportation, DME, medications, and referrals to outside services; collaborate with payer and community case management programs to ensure effective transitions of care; addresses patient, family customer service opportunities timely; schedule new employee orientation, determine preceptor, monitor progression, and evaluate the new hire at 30, 60, 90 days and as needed. Ensure competency documentation is in place; organize educational opportunities for case management department based upon staff needs and case management innovation; communicates and uses appropriate customer relation skills with physicians, patients, families, and healthcare team in all methods of communication; responds to all forms of communication timely and meets the demands of the organization; accepts and completes all duties positively and without conflict; cooperates, helps others and improves the performance of the department; completes all mandatory department, educational and hospital requirements; utilizes cost effective practices in performing all aspects of the job; maintains orderliness and cleanliness of work areas, equipment and supply areas; adheres to current Infection Control and Safety Standards; provides assessments and interventions of care for all ages as related to social services and discharge planning; continue to fulfill job functions of specific position held and cover as social worker as needed.

Qualifications

The minimum training and experience requirements for this classification are master’s degree in social work from an accredited institution as approved and accepted by the University of South Alabama, five years of experience including both behavioral and medical health, and licensure with the State of Alabama as a Social Worker. Knowledge in both the adult and pediatric setting preferred.
