Pay Rate

MSW $35/hr-$40/hr;

LMSW $40/hr-$45/hr;

LCSW $45/hr-$50/hr.

Full Description

VitalCheck Wellness, on behalf of Photon Medical Group, is seeking a culturally sensitive and compassionate Social Worker to join our team. Licensed Clinical Social Worker (LCSW), Master of Social Work (MSW), or Licensed Master Social Worker (LMSW) are encouraged to apply. Patient care will be provided in-person and virtually at one of NYC Health + Hospitals’ Humanitarian Emergency Response and Relief Centers (HERRCs) making a difference in the lives of migrants.

Responsibilities

• Conduct culturally competent short-term counseling, crisis intervention, risk assessment, safety planning, and psycho-education, including:
• Crisis intervention
• Identify need and make referrals, for outpatient/inpatient care and hospital transport
• Trauma triage/trauma informed assessment
• Peer support
• Conducting emergency evaluation/assessment for:
• Mental health and mental health crises
• Substance use/disorder
• Occupational needs
• Domestic violence
• Advocacy on behalf of the family when working with NYC Administration for Children’s Services or other preventative services.
• Communicating and collaborating with external service providers to enhance engagement and ensure quality of service delivery.
• Providing education and training to other staff on mental health and holistic support needs and benefits.
• Facilitating family meetings with the family and HERRC staff to discuss planning efforts and progress.
• Coordinating with emergency services, hospital social workers, and discharges, particularly around laboring individuals and newborns.
• Coordinating on domestic violence cases, particularly in instances where a 911 call must be made.
• Handling transfers to domestic violence shelter.
  

Requirements

• Master's in Social Services, Mental Health Counseling, and/or Marriage and Family Therapy.
• Licensed Clinical Social Worker (LCSW), Master of Social Work (MSW), or Licensed Master Social Worker (LMSW) certification.
• Experience in a healthcare or hospital setting is highly preferred.
• Knowledge of New York State regulations and guidelines for social work practice.
• Strong interpersonal and communication skills.
• Experience in community-based services, shelters, immigrants; volunteer or mission experience with disaster relief, refugee, humanitarian relief, preferred.
• Empathy and sensitivity to the needs of diverse patient populations.
• Bilingual in Spanish a Plus.
• Strong interpersonal and communication skills.
• Ability to work effectively and independently in a fast-paced and dynamic environment.
• Proficiency with computers and electronic medical records systems.
• Must be able to commute to New York City and Boroughs.
