Social Work Services at the Mount Sinai Hospital

The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System’s implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, workflow optimization, physician engagement and quality reporting. The Department of Social Work Services takes a leadership role in MSHP Care Management, which is providing in person and telephonic care management services for a large diverse group of individuals who have a variety of insurances that are part of value based contracts. MSHP Care Management engages in a patient-centered approach, which aims, through improved care coordination and service integration, to control future health care costs and improve health outcomes for this population.

Responsibilities

• The role of the Social Worker within MSHP Care Management is to serve as a member of the interdisciplinary care team and to work closely with this team to provide complex care management services to patients at risk for avoidable hospital and emergency department utilization.
• The Social Worker will conduct biopsychosocial assessments with patients and family caregivers, identify barriers to seeking medical/behavioral health care and work with the team and patient/family caregivers to develop a care plan to overcome these barriers and improve health outcomes with the goals of reducing readmissions and facilitating transitions of care.
• They will create linkages for patients with community organizations to facilitate adherence to the care plan and educate patients/family caregivers on how to navigate the health system.
• For common patients, the Social Worker communicates with social workers/care coordinators within MSHP and the Health System to ensure collaborative and seamless work.
  
  

This position will be located at MOUNT SINAI DOCTORS LONG ISLAND in Greenlawn, Suffolk County. The successful applicant will be onsite 3-5 days per week and work in collaboration with a Care Coordinator who is also on site.

Qualifications

• LMSW or LCSW, or Limited Permit Eligible.
• Exceptional skills in engaging patients/families and utilizing critical thinking skills in assessment and coordination of resources within families and hospital and community networks.
• Ability to develop, maintain and monitor connectivity to medical/behavioral health care, safety and wellbeing of patients.
• Exceptional organizational, collaborative and psychosocial assessment and intervention skills.
• Ability to foster a supportive environment for patients and colleagues, and work collaboratively as part of a team.
• Ability to be creative in identifying solutions that drive optimal health for patients and families
• Consistent adherence to established clinical and operational protocols aimed at promoting measureable clinical outcomes.
• Interest in and passion for being part of an innovative community-based program to improve the quality of care by addressing barriers to behavioral health, primary and specialist care, and avoid unnecessary Emergency Department visits and hospital admissions/readmissions.
• Healthcare and/or community based experience working with patients with complex chronic illness and/or behavioral health population preferred.
• Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications.
• Bilingual (English/Spanish) preferred.

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $81999.92 - $94759.89 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
