Job Summary

This dual position is approximately 60% rural health clinic based and 40% home hospice based. In the clinic, the social worker is responsible for providing clinical social work services to patients and families and acting as an advocate to help clients to access resources while confronting their personal issues such as mental illness, addiction and abuse. Provides individual psychotherapy, family therapy and group therapy when appropriate and as needed. In the hospice setting, the social worker collaborates, communicates and facilitates coordination of services. The social worker performs an in-home intake assessment and ongoing in-home visits as directed by the patient’s plan of care and state licensure requirements. Additionally, the social worker facilitates grief support groups for adults and children. This position reports to the Director of Physician Practice Administration & Operations.

Responsibilities

  • Complete psychosocial assessment on patients/family/support situations including social, psychological, emotional, financial, and other relevant factors to facilitate discharge planning.
  • Participate in daily multidisciplinary health care team meeting, provide relevant information in relationship to patient discharge readiness
  • Maintain regular contact with patients, families, and caregivers regarding anticipated needs, plans, date, treatment options, resources, and assistance, etc.
  • Complete all required choice forms, discharge notices, etc., per regulations.
  • Maintain current knowledge of available post hospital resources; and maintain adequate supply of resource materials for patients, families, and caregivers.
  • Provide education to patients, families, caregivers on Kansas Advance Directives.
  • Ensure pertinent medical information at time of discharge is provided to receiving providers to affect a smooth transition/continuity of care.
  • Complete reports to Protection Report Center on suspected patient abuse, neglect, and or exploitation.
  • Plan daily routine to properly organize tasks and demonstrate willingness to meet immediate patient needs effectively and efficiently.

Job Requirements

  • Masters Degree in Social Work (MSW)
  • Minimum of one year experience in case management/care coordination/discharge planning.
  • Candidate must hold current license as a Social Worker in the state of Kansas.
  • BLS certification required or much complete BLS certification within the first two weeks of employment.
