Social Worker Job, Full-Time, Patient Centered Medical Home

Nashville General Hospital is hiring for a Social Worker for Patient Centered Medical home as a full time job in Nashville, Tennessee.

Shift: Day Shift

The Social Worker facilitates partnerships between patients, physicians and health care teams, focusing on care coordination and integration of treatment plans. The Social Worker tracks and supports patients when they obtain services inside and outside the clinic, and ensures safe and timely referrals or transitions by deliberately integrating patient care activities between two or more members of the interdisciplinary team involved in a patient’s care. The main objective of the Social Worker is to assess and evaluate patients social, psychological, physical and economic needs and to facilitate the appropriate delivery of services to assist patients in meeting these needs to reduce fragmented care.

Requirements

• Bachelor's in Social Work (Masters preferred)
• 3-5 years in Healthcare Social Work
• Previous experience working with adult population within ethnically and culturally diverse families.
• Social Work in an Ambulatory setting preferred
