Social Worker Salary: $23.00 - $24.00 per hour based on experience.

Our long-term care facility is seeking a dedicated and experienced Social Worker. As a Social Worker, provide medically related social services that assist the residents to attain or maintain the highest practicable physical, mental, and social well-being.
Social Worker Key Duties
Conducts pre-admission screening of individuals seeking admission to the facility, including persons with mental illness according to facility admission criteria.
Completes and updates social services assessments for residents.
Function as Care Plan Coordinator as assigned.
Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plans of care to protect and promote residents’ rights.
Records progress notes in the clinical record. Observations of resident’s behavior and interventions
Perform other job-related duties as they become required.
Social Worker Qualifications
Bachelor’s degree required.
LSW licensure required; LCSW or LICSW required.
Minimum of 2 years’ experience in long-term care or a related setting.
Knowledge of local, state, and federal regulations and Dept. of Health Survey process.
Ability to work effectively as part of a multidisciplinary team.
