The Social Services Coordinator shall have primary responsibility for the assessment, planning, and access coordination of medical, psychosocial programs and/or other social needs. Collaborates with Behavioral Health and Primary Care providers, and other members of the patient’s interdisciplinary care team in evaluating patient progress against the care plan; reviews and communicates with patients revisions/updates of plan and/or goals.

Must meet one of the following qualifications

• Bachelor of Health and Human Services-related Degree from an accredited school of social work
• Current Nevada State Social Work Licensure(LSW) Preferred
  
  
  

Additional Qualifications

• Bilingual Spanish and English preferred
• Minimum one year work experience in an outpatient (or Inpatient) behavioral health setting preferred
