The Community Access Service Coordinator manages processes for enrollments and transitions for participants in the Texas Home Living and Home and Community Based Services programs. The Service Coordinator works under the supervision of the Community Access Team Leader, IDD Authority Services Division. The IDD Service Coordinator is responsible for the following:
Performing processes for waiver enrollments, transfers and reassignments.
Provision of Medicaid billable targeted case management activities, i.e., assessment, service planning, monitoring, advocacy and crisis prevention and management for individuals with IDD
Compliance with applicable local, state and federal rules and regulations.
Adherence to and promotion of the Texana Compliance Policy.
Travel within 6 county region.
Work hours: M-F, 8-5 with flexibility based on client need.
Salary
$19.72 to $24.24/hour based on years of experience performing Medicaid Targeted Case Management
Strong organizational and computer skills (Microsoft Office and Electronic Health Record system).
Knowledge of Service Coordination State Rules.
Experience working with individuals with IDD.
Job related Physical Requirements
Must be able to operate a vehicle
Must be able to perform SAMA and CPR. Training is provided to employees once hired
Must be able to perform moderate to heavy lifting (15#- 45#), pushing, stooping, light carrying (15#), repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting
Must have ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
