Job Title: Service/Care Coordinator

Location: Remote Dallas, TX. The SC will be conducting home visits 3 days a week in the Dallas area. There is not an office, they will be working remotely.

Duration: 3 Months

Shift Hours: 8:00am to 5pm CST, no overtime required.

Summary

  • They will need to review emails and tasks for the day.
  • Contact members if needed to confirm home visits.
  • Ensure all follow-up is completed for members and their services are in place.
  • The expectations are to complete visits and tasks on time.
  • Some that has worked as a Services Coordinator with SHP in the past. That has completed training.
  • LVN/LPN or Social Worker
